Objective:Analyze the correlation between commonly used hematocrit(HCT)and serum albumin(ALB)and patient infection and severity,and evaluate their value in predicting patient infection and prognosis of infection.Methods:Selected patients with chronic kidney disease that underwent hemodialysis(150people in total)who was admitted to Ma’anshan shiqiye Hospital from September 2020 to August 2021 were divided into a case group and a control group(75 people each).The blood samples drawn at the first admission without blood product transfusion are marked as HB,HCT,ALB,Cys C,Fe,total CO2 binding capacity level and HCT-ALB value,etc.,and compare the case group with the control group,compare the death(12 people)with the survival(63 people)in the case group,and compare the difference between HCT and ALB data,and whether the patients with chronic kidney disease undergoing hemodialysis at stage 5 are co-infected Perform statistical analysis,draw ROC curve,and find the best positive likelihood ratio index.Results:(1)HB and HCT are statistically significant in whether MHD patients are co-infected(t=-3.484,p=0.001;t=-3.783,p=0.000);(2)Maintenance hemodialysis patients without infection are better than MHD patients with co-infection The overall values of hemoglobin and hematocrit are relatively high;(3)HCT-ALB,|HCT-ALB|,the difference between the infection group and the control group is more obvious(t=-5.350,p=0.001;t=5.675,p=0.000);In the infection group,comparing the indicators of dead and surviving patients,it was found that the difference in indicators between the two groups of HCT,ALB and |HCT-ALB| was statistically significant,and the HCT and ALB values of patients who died of infection were less than those of patients who survived infection;(5)|HCT-ALB| is used to identify whether patients with chronic kidney disease stage 5 hemodialysis have co-infection.The area under the ROC curve is 0.830,the corresponding standard error is 0.052,P=0.000,and the 95% confidence interval is(0.729,0.931),the study found that the best diagnostic value was 8.20,the sensitivity at this point was 0.733,and the specificity was 0.90;(6)Other indicators in this experiment,such as Cys C,Fe,and total CO2 binding capacity levels,have different mean values.The difference was not statistically significant(P>0.05).Conclusion:HB,HCT,and the difference between HCT and ALB are statistically significant in evaluating whether patients with chronic kidney disease undergoing hemodialysis at stage 5are co-infected.And in the infection-fatal group,the above differences are more significant,suggesting that HB,HCT,and the difference between HCT and ALB have a certain value in predicting infection and prognosis of MHD patients.
