Objective:To investigate the relationship between the different stages of diabetic retinopathy(DR)and macular choroidal volume(MCV)by Swept-Source Optical Coherence Tomography.To analyze the clinical significance of MCV in early detection and early treatment of diabetic retinopathy.To observe the macular choroidal volume in patients with diabetic macular edema(DME)after intravitreal injection of anti-vascular endothelial growth factor(VEGF)drugs,analyzed the effect of drug treatment,and the characteristics of changes in the macular choroidal volume.Methods:The study was a prospective,non-randomized controlled study.All subjects were diagnosed as diabetic retinopathy groups(72 cases,91 eyes)and diabetic macular edema groups(16cases,16 eyes)according to the presence or absence of macular edema in the ophthalmology departments of Hunan Provincial People’s Hospital from June 2020 to December 2020.Subjects who were in good physical condition were involved as a normal control group(20 cases,20 eyes).The clinical experimental group for diabetic retinopathy,conducted by two experienced and professionally qualified doctors,was divided as follows:Non-diabetic retinopathy(NDR)group(18 cases,24 eyes).Nonproliferative diabetic retinopathy(54 cases,67 eyes): mild NPDR group(20 cases,26 eyes),moderate NPDR group(21 cases,24eyes),and severe NPDR group(13 cases,17 eyes).Patients with diabetic macular edema and eligible for intravitreal injection procedures were recruited(16 cases,16 eyes)and MCV was collected from all patients one day preoperatively,2 weeks postoperatively,and 1 month postoperatively.Patients with DME were administered intravitreal injections using the same anti-VEGF drug.All patients underwent endocrine specialist examination and ophthalmic visual acuity,intraocular pressure,ocular axis,fundus photography,slit-lamp examination,and sweep-frequency optical coherence tomography examination,and all subjects met the inclusion and exclusion criteria.Results:1.There was a negative correlation between age and MCV,the older the age,the smaller the MCV.The difference was statistically significant(P < 0.05).2.There was no significant correlation between gender factors and macular choroidal volume(p > 0.05).3.The MCV was significantly larger in the normal control group than in the experimental diabetic group(p < 0.05).4.The MCV was greater in the normal group than in the mild NPDR group,the moderate NPDR group,and the severe NPDR group,with a statistically significant difference(P<0.05).The MCV was significantly smaller in the severe NPDR group compared to normal controls,NDR group,mild NPDR group,and moderate NPDR group,with a statistically significant difference(P<0.05).The MCV was reduced in the moderate NPR group compared to the mild NPDR group,the NDR group,and the normal control group,with a statistically significant difference(p<0.05).There was a statistically significant difference in MCV between the mild NPDR group and the normal control group(P<0.05).NDR group and normal control group,and between groups Mild NPDR NDR Pairwise comparison MCV showed no statistically significant difference(P>0.05).5.The MCV with DME patients at the 2 weeks postoperatively and the 1 month postoperatively was significantly smaller than those before injection,and the difference was statistically significant(P < 0.05).Conclusion:1.The macular choroidal volume was closely correlated with the severity of DR,suggesting that MCV could be used as a detection indicator for DR.2.In the early stage of DR,patients have already experienced a decrease in MCV,and with further aggravation of the disease,MCV further decreases,suggesting that MCV could be used as a detection indicator for early detection of DR and disease intervention.3.After intraocular injection of anti-VEGF drugs in patients with diabetic macular edema,MCV decreased at 2 weeks and 1month after surgery,suggesting that MCV could be used as a detection index for the analysis of drug efficacy after intravitreal injection of anti-VEGF drugs.
