| ObjectiveTo explore the expression level and clinical significance of serum Beclin1 and LC3 in patients with bronchial asthma by detecting the concentration of serum autophagy-associated protein Beclin1 and microtubule-associated protein 1 light chain3(LC3).Methods105 patients with acute attack of bronchial asthma hospitalized in the Second Affiliated Hospital of Shenyang Medical College from December 2020 to December2021 were collected as the asthma group,including 35 patients with bronchial asthma with eosinophil(EOS)count lower than 0.05(109 / L)and 35 patients with eosinophil(EOS)reduction asthma group;35 patients with bronchial asthma with EOS count between 0.05 and 0.5(109 / L)were in EOS normal asthma group;35 patients with bronchial asthma with EOS count higher than 0.5(109 / L)were asthma group with elevated EOS;35 healthy persons who came to the physical examination department of our hospital for physical examination at the same time were collected as the control group.First,the basic information of all subjects was recorded,and the serum concentrations of Beclin1 and LC3 were detected by enzyme linked immunosorbent assay(ELISA).SPSS 26.0 statistical software was used to analyze the data of each group,and the basic clinical data and serum Beclin1 and LC3 levels of patients in each group were compared.Binary logistic regression analysis was performed on serum Beclin1 and LC3,and receiver operating characteristic curve(ROC)was drawn.Combined with area under the curve(AUC)and Jordan index,the predictive value of the two factors for asthma attack was judged.The correlation was analyzed by Pearson method,and the scatter plots of serum Beclin1,LC3 and EOS levels in the asthma group were drawn respectively.(the standard is α = 0.05,P < 0.05,indicating that the difference is statistically significant)Results(1)According to the comparison of general clinical data(including age,sex,height,weight,BMI),there was no significant difference between EOS decreased asthma group,EOS normal asthma group,EOS increased asthma group and control group(P>0.05).(2)There was no significant difference in C-reactive protein(CRP),neutrophil count(N),neutrophil percentage(N%)and white blood cell(WBC)among EOS decreased asthma group,EOS normal asthma group and EOS elevated asthma group(P>0.05).The EOS counts in EOS decreased asthma group,EOS normal asthma group and EOS increased asthma group were 0.03(0.01,0.04),0.26(0.19,0.31)and 0.61(0.51,0.79)respectively.The mean values of the three groups were not all equal,and the differences were statistically significant(P < 0.05).(3)The serum Beclin1 expression level of the asthma group and the control group was 7.05(6.29,7.70)and 5.26(4.95,5.49)respectively.The serum Beclin1 expression level of the asthma group was significantly higher than that of the control group(P < 0.05),The expression level of serum LC3 in the asthma group and the control group was 1355.36(1219.10,1520.71)and 1056.15(1032.06,1136.89)respectively.The expression level of serum LC3 in the asthma group was significantly higher than that in the control group(P < 0.05).(4)The levels of serum Beclin1 in EOS decreased asthma group,EOS normal asthma group,EOS increased asthma group and control group were 6.22 ± 0.63ng/ml,6.99 ± 0.58ng/ml,8.00 ± 0.82ng/ml and 5.29 ± 0.57ng/ml respectively.The mean values among the four groups were not all equal,and the levels of serum Beclin1 in the three asthma groups were significantly higher than those in the control group,and the levels of serum Beclin1 in EOS increased asthma group and EOS normal asthma group were significantly higher than those in EOS decreased asthma group(P < 0.05),The serum Beclin1 level in EOS elevated asthma group was significantly higher than that in EOS normal asthma group(P < 0.05).The serum LC3 levels of subjects in EOS decreased asthma group,EOS normal asthma group,EOS increased asthma group and control group were 1170.42 ± 110.52pg/ml,1341.69 ± 133.27pg/ml,1559.61 ±98.69pg/ml and 1066.63 ± 94.94pg/ml respectively.The mean values among the four groups were not completely equal,and the serum LC3 levels of the three asthma groups were significantly higher than those of the control group,and the serum LC3 levels of EOS increased asthma group and EOS normal asthma group were significantly higher than those of EOS decreased asthma group(P < 0.05),The level of serum LC3 in EOS asthma group was significantly higher than that in EOS normal asthma group(P <0.05).(5)Logistic regression analysis showed that serum Beclin1 and LC3 were independent predictors of asthma attack(or value: Beclin1 = 9.592;LC3 = 1.007;P <0.05).Then the ROC curve was used to evaluate the predictive value of the two factors for asthma attack.It can be seen that the AUC of serum Beclin1 for predicting asthma attack was 0.938(95% CI: 0.898-0.979,P < 0.05),the Jordan index,sensitivity and specificity were 0.819,0.905 and 0.914 respectively,and the critical value was5.923ng/ml;The AUC of serum LC3 in predicting asthma attack was 0.893(95% CI:0.841-0.945,P < 0.05).The yoden index,sensitivity and specificity were 0.790,0.819 and 0.971 respectively,and the critical value was 1200 pg / ml.The AUC of serum Beclin1 and LC3 in predicting asthma attack was 0.953(95% CI: 0.921-0.986,P <0.05),and the yoden index,sensitivity and specificity were 0.828,0.857 and 0.971 respectively.(6)Pearson analysis showed that the expression levels of serum Beclin1 and LC3 in patients with asthma were positively correlated with the level of eosinophils in blood,and the correlation was significant(r = 0.722,0.809,P < 0.05).The expression levels of serum Beclin1 and LC3 were also positively correlated,and the correlation degree was significant(r = 0.769,P < 0.05).ConclusionsThe expression levels of serum Beclin1 and LC3 in patients with bronchial asthma were significantly increased,and both were positively correlated with the level of EOS in blood,it is suggested that the increase of serum Beclin1 and LC3 levels can predict the acute attack of bronchial asthma. |
