| Background and Objective:The WHO cited the increase in the prevalence of diabetes mellitus and DM as the "twenty-first Century epidemic".China has become the country with the largest number of adults suffering from DM in the world.Patients with type 2 diabetes mellitus(T2DM)are often accompanied by obesity.Visceral fat(VF),as an evaluation index of obesity,participates in the occurrence and development of T2 DM and plays a role that cannot be ignored.The number of T2 DM patients complicated with hyperuricemia(Hua)is also increasing.Studies have shown that obesity is an independent risk factor for Hua.Some studies showed that the positive correlation between serum uric acid(SUA)and insulin resistance index(IR)in T2 DM patients maybe at least partially or completely mediated by central obesity.VFA as an important index for evaluating central obesity,is significantly correlated with SUA level.The purpose of this study was to investigate the relationship between VFA and T2 DM complicated with HUA,as well as the risk factors of T2 DM complicated with HUA.Thus,it can effectively reduce the prevalence of T2 DM patients with HUA,prevent the corresponding complications caused by HUA,and control and delay the occurrence and development of T2 DM.Methods:Through the electronic medical record system,150 patients who were diagnosed as T2 DM and met the requirements in the inpatient department of Endocrinology Department of China Japan Friendship Hospital of Jilin University from June 2021 to December 2021 were selected retrospectively.According to SUA,79 patients were divided into T2 DM group(SUA≤420μmol/L)and T2 DM combined with HUA group(SUA > 420μmol/L).Collect the general data of the subjects: name,gender,age,height, weight and BMI;Biochemical indexes:serum uric acid(SUA),fasting insulin(fins),fasting plasma glucose(FPG),glycosylated hemoglobin(Hb A1c),triglycerides(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),Low density lipoprotein cholesterol(LDL-C)and serum homocysteine(Hcy).Measure visceral fat area and subcutaneous fat area.Calculate insulin resistance and body mass inde.The general data and biochemical indicators of the two groups were compared,and the risk factors of T2 DM complicated with HUA were analyzed,as well as the diagnostic significance of VFA in T2 DM complicated with HUA.Results:1.Age and VFA of T2 DM combined with HUA group were higher than those of T2 DM alone group,the differences were statistically significant(P < 0.05).There were no significant differences in gender,BMI,smoking history,SFA and drinking history between the two groups(P > 0.05).2.FINS,HOMA-IR and TG in T2 DM group with HUA were higher than those in T2 DM group without HUA,the differences were statistically significant(P< 0.05).There were no significant differences in FPG,Hb A1c%,TC,LDL-C,HDL-C and HCY between the two groups(P > 0.05).3.Taking the combination of HUA as the dependent variable,single factors with statistical significance in the comparison of general data and clinical indicators were included in binary Logistic regression analysis for risk factors of T2 DM with HUA.The results showed that HOMA-IR and VFA may be independent risk factors for T2 DM with HUA(P < 0.05).4.ROC curve was drawn with the presence or absence of HUA as the state variable and VFA as the test variable.The area under the ROC curve for VFA diagnosis of T2 DM with HUA was 0.705,with a standard error of 0.043,P< 0.000 compared with 0.5,and the 95% confidence interval of area was(0.621,0.788).The Calculated Yuden index was 0.327,and the VFA critical value for predicting HUA in T2 DM patients was 97cm~2,the specificity was 0.750,and the sensitivity was 0.577.Conclusion:1.VFA is correlated with SUA level in T2 DM patients,and VFA is an independent risk factor for T2 DM complicated with HUA,2.T2 DM patients with VFA≥97cm~2 should be alert to the occurrence of HUA,with specificity of 0.750 and sensitivity of 0.577. |
