Objective:To analyze the correlation between blood glucose fluctuation index SDBG and the formation of lower extremity atherosclerotic plaque in type 2 diabetes.Methods:Epidata software was used to collect the relevant information of diabetic patients hospitalized in the Department of Endocrinology of the First People’s Hospital of Hefei,and the relevant information of patients with type 2 diabetes was screened out,including the general data of patients(gender,age,course of disease,smoking history,drinking history,family history,systolic blood pressure,diastolic blood pressure,body mass index),laboratory indexes(triglyceride,cholesterol,high density lipoprotein,low density lipoprotein,glycosylated hemoglobin,fasting blood glucose,fasting C peptide)and related auxiliary examinations(lower extremity arterial color Doppler ultrasound).According to the inclusion and exclusion criteria,a total of 1040 patients with type 2diabetes hospitalized in the endocrinology department of Hefei First People’s Hospital from January 2015 to December 2016 were selected for retrospective study.According to the results of lower extremity arterial color doppler ultrasound(lower extremity arterial intima-media thickness),the screened patients with type 2 diabetes were divided into plaque group and non-plaque group.There were 687 cases in plaque group and 353 cases in non-plaque group,including 561 males and 479 females.According to the measured fingertip blood glucose,the mean value of standard deviation of blood glucose(SDBG)within three days before admission was calculated to evaluate the blood glucose fluctuation level.According to the expert consensus on blood glucose fluctuation management,the normal SDBG value should be controlled within 2 mmol/l.According to the SDBG value,the patients were divided into the normal blood glucose fluctuation group(0≤SDBG<2 mmol/l,n=133)and the abnormal blood glucose fluctuation group(SDBG≥2 mmol/l,n=907).Single factor analysis and binary logistic stepwise regression analysis were performed to assess the effect of blood glucose fluctuation on plaque formation of lower limb atherosclerosis in patients with type 2 diabetes.Results:There was no significant difference in gender,smoking history,drinking history,diastolic blood pressure,cholesterol,low density lipoprotein,glycosylated hemoglobin,fasting blood glucose,fasting C peptide,and body mass index between the two groups(P>0.05).Compared with the non-plaque group,the age of the plaque group was older,the systolic blood pressure was higher,the high-density lipoprotein was higher,the course of disease was longer,and the triglyceride was lower.The differences in family history and blood glucose fluctuation between the two groups were statistically significant(P<0.05).The blood glucose fluctuation level was correlated with the plaque formation of lower limb atherosclerosis(?~2=6.356,P<0.05).Univariate analysis of significant variables combined with clinical,significant variables for the progress of atherosclerosis were included in the predictive factors,including age,gender,course of disease,smoking history,systolic blood pressure,diastolic blood pressure,triglyceride,cholesterol,low density lipoprotein,high density lipoprotein,fasting blood glucose,glycosylated hemoglobin,SDBG.With plaque in the lower extremity as the dependent variable,binary Logistic stepwise regression analysis showed that age,gender,systolic blood pressure,SDBG were independent risk factors for the formation of atherosclerotic plaque in the lower extremity(P<0.05).The difference between abnormal blood glucose fluctuation group and normal blood glucose fluctuation group was statistically significant OR=1.653(95%CI:1.086-2.516,P=0.019).The level of blood glucose fluctuation was significantly correlated with lower extremity atherosclerosis.Conclusion:There was a correlation between blood glucose fluctuation and plaque formation in type 2 diabetes mellitus.In the daily diagnosis and treatment work,timely detection and control of abnormal blood glucose fluctuations can delay the occurrence and progress of diabetic vascular complications.It is necessary to comprehensively judge the patient’s condition,comprehensively consider and formulate corresponding hypoglycemic schemes to achieve stable blood glucose control.
