Clinical Study On Relationship Between TCM Syndrome Differen Tiation Of Type2 Diabetes Combined With Hypertension And Thy Roid Function And Autoimmue Thyroid Antibodies

Objective To study the relationship between TCM syndrome differentiation of Type2 Diabetes Combined With Hypertension(DH)and thyroid function and autoimmue thyroid antibodies.Methods According to WHO diagnostic criterion of western medicine and diagnostic criterion of TCM and criterion of exclucive case,30 cases of each syndrome of DH were selected,including Yin deficiency and excess heat syndrome(YDEHS),Damp Heat syndrome(DHS),Qi and Yin deficiency syndrome(QYS),Stagnated blood syndrome(SBS)and Deficiency of Yin and Yang syndrome(DYYS).30 cases of healthy people were selected as the normal control group(NCG).BMI,HbA1c,FT3,FT4,TT3,TT4,TSH,TPOAb and TGAb were measured.The relationship between the indexes and syndrome differentiation of TCM were analyzed.Resluts ① Among DH patients,BMI of DHS was higher than other four syndromes and NCG(P<0.05 or P<0.01).② The comparison of course of disease,HbA1c,FPG among all TCM syndromes showed that SBS and DYYS were significantly longer or higher than other three syndromes(P<0.05 or P<0.01).③ The prevalence of hyperthyroidism rate between DH group and NCG was no significant difference(P>0.05),the comparison of the prevalence of hypothyroidism rate between them was statistically significant(P<0.05).④ Among DH patients,the prevalence of thyroid disfunction of female is higher than that of male(P=0.012),especially of hypothyroidism(P=0.023)⑤ The prevalence of hyperthyroidism between all syndromes of DH were no significant difference(P>0.05),the prevalence of hypothyroidism of DYYS is higher than other four syndromes(P<0.05).⑥FT3 of YDEHS,DHS and QYS were higher than SBS and DYYS(P<0.05 or P<0.01);FT4 of DYYS was lower than other four syndromes(P<0.05),TSH of DYYS was significantly higher than other four syndromes(P<0.01).⑦ The positive rates of TPOAb among all TCM syndromes were significant difference(P<0.05),the positive rates of TPOAb of YDEHS is higher than other four syndromes(P<0.05).The positive rates of TGAb among all TCM syndromes were no significant difference(P>0.05).Conclusions ① Among DH patients,BMI of DHS was highest and the degree of obesity in DHS is most severe.②The course of disease,HbAl c and FPG of SBS and DYYS were significantly longer or higher than other three syndromes,which showed that the level of blood sugar of SBS and DYYS were poorer.③ The prevalence of hypothyroidism rate of DH is higher than NCQ the prevalence of thyroid disfunction of female is higher than that of male among T2DM patients,especially the prevalence of hypothyroidism.④ Among all TCM syndromes,the prevalence of hypothyroidism of DYYS is higher than other syndromes.It showed that thyroid function can reflect to a certain extent the severity of DH,FT3 can be used as a measure of DH glycemic control.⑤ The positive rates of TPOAb of YDEHS is higher than other syndromes.It should routinely exam TPOAb in the YDEHS,promoting early diagnosis,prevention and treatment of hypothyroidism.
