Objective:To evaluate the value of neutrophil to lymphocyte ratio(NLR)in distinguish large-artery atherosclerosis(LAA)and small vessel occlusion(SVO).Methods:From January 2012 to August 2016 consecutive patients with first onset acute stroke registered in the Nanjing Stroke Registration System were included.Using the National Institutes of Health Stroke Scale(NIHSS)to assess the severity of the patients.Patients with an NIHSS<3 were selected.SPSS23 software was used to evaluate the relationship between NLR and LAA classification of the single factor analysis and binary logistic regression analysis.Results:In univariate analysis,white blood cell count,neutrophil,lymphocyte,NLR,glycosylated hemoglobin,HDL,time from stroke onset to treatment(OTT),time from stroke onset to NLR test,are associated with the LAA classification.In binary logistic regression,NLR,time from stroke onset to NLR test,glycosylated hemoglobin are independent factors of LAA.The ROC area of NLR predicting LAA for patients arrived within 7 days is 0.611 and 0.673 for patients who arrived within 24h.Conclusions:NLR,time from stroke onset to NLR test,glycosylated hemoglobin are associated with LAA classification independently.The NLR of LAA classification is higher than SVO classification.The NLR predicts that the area under the ROC curve of LAA is small.In 7 days and the predictive value of TOAST classification is limited.Within 24 hours,the area under the ROC curve is higher than the ROC curve of the patient within 7 days.The earlier NLR maybe more valuable in predicting the LAA classification.
