Objective:Through meta-analysis,the influencing factors of treatment compliance of bronchial asthma patients were clarified,and later intervention measures were provided to provide research data and reference basis;an intervention plan was established to evaluate the effect of full-time nurse-led individualized health education on treatment compliance of asthma patients.Method:1.Systematic search of Chinese Journal Full-text Database(CNKI),Wanfang Database,Weipu Chinese Journal Full-text Database(VIP)and Chinese Biomedical Literature Database(CBM)and other Chinese databases published in 2009 to December 2019 on bronchial asthma patients A cross-sectional study of the influencing factors of inhalation compliance,and screening of the included literature.Three researchers conducted a rigorous evaluation of the quality of the literature and data extraction,and finally included 14 articles that met the quality standards and analyzed the content.2.Full-time nurse-led personalized health education on the treatment compliance of asthma patients.Using convenience sampling method,92 cases of bronchial asthma patients who were revisited in the asthma clinic of the First Affiliated Hospital of Guangxi Medical University from July 2018 to September2019 were selected.45 cases in the control group and 47 cases in the intervention group.The control group was given routine treatment and care,and the intervention group was given individualized health education on the basis of routine treatment and care.Before and after the intervention,the Morisky medication compliance scale,ACT scale,asthma knowledge mastery scale,regular follow-up,Write an asthma diary regularly for evaluation.Result:1.Meta analysis results of factors influencing treatment compliance of asthma patients: A total of 13 studies were included,with a sample size of 2290 cases,992 cases with good treatment compliance,1298 with poor treatment compliance,and 56.7% with poor treatment compliance.The scores of literature quality evaluation are all ≥4 points,all of which are of medium quality or above.Inhalation technique10.47[3.02,36.24],asthma knowledge 2.61[1.17,5.80],age 0.50[0.28,0.91],education level 0.49[0.28,0.86],doctor-patient relationship0.34[0.12,0.95],The severity of the disease was 0.25 [0.11,0.58],and adverse drug reactions 0.23 [0.13,0.41] were the influencing factors of treatment compliance.2.The effect of personalized health education intervention led by a full-time nurse:(1)Comparison of correct inhalation techniques between the two groups of patients: the first evaluation of the correct rate of inhalation techniques between the two groups of patients,no statistical difference(P>0.05),3 months after intervention There was no statistical difference between the control group and the first evaluation(P>0.05),the intervention group was higher than the control group and the first evaluation of the same period,there was a statistical difference(P<0.05).(2)Comparison of asthma disease knowledge scores between the two groups of patients: There was no statistical difference in the scores of asthma disease knowledge between the two groups before intervention(P>0.05).After 1 month and 3 months of intervention,the scores of the intervention group and the control group Both were higher than before the intervention,and the score of the intervention group was significantly higher than the control group of the same period,with statistical difference(P<0.05),there was an interactive effect between the groups and time(P<0.05),and the time factor was significant(P<0.05),Over time,the scores of patients in the intervention group gradually increased.(3)Comparison of medication compliance between the two groups of patients: There was no statistical difference in medication compliance between the two groups after 1 week of treatment(P>0.05).After 1 month of intervention,there was no statistical difference between the control group and the intervention group(P>0.05).After3 months of intervention,the compliance of the control group decreased significantly with statistical difference(P<0.05);the intervention group was higher than the control group of the same period,the difference was statistically significant(P<0.05).(4)Comparison of self-management compliance between the two groups of patients: After 3 months of intervention,the patients in the intervention group recorded asthma diary compliance and follow-up compliance were higher than the control group,with statistical differences(P<0.05).(5)There was no statistical difference between the ACT scores of the two groups before intervention(P>0.05).After 1 month of intervention,there was no statistical difference between the control group and the intervention group(P>0.05),after 3 months of intervention,The score of the control group decreased,and the score of the intervention group increased,there was a statistical difference(P<0.05).There was an interactive effect between the groups and time(P<0.05),and the time factor was significant(P<0.05).As the time increased,the scores of patients in the intervention group gradually increased.(6)Pulmonary function comparison between the two groups: There was no statistical difference between the two groups before the intervention(P>0.05).After 3 months of intervention,the control group and the intervention group were statistically different than before the intervention(P<0.05),the improvement of the intervention group was better than the control group of the same period,with statistical difference(P<0.05).Conclusion: Asthma disease knowledge and inhalation technology are risk factors for patient compliance;age,education,doctor-patient relationship,mild disease severity,and no adverse drug reactions are protective factors.Personalized health education led by a full-time nurse helps patients to correctly master inhalation technology,improve asthma disease knowledge,record asthma diary compliance and follow-up compliance,maintain medication compliance at a high level,improve patients’ asthma control and lung function.
