ObjectiveBy reviewing a diagnosis of diabetes peripheral neuropathy patient’s four diagnostic data and ABI,lower limb artery colour to exceed,indexes of blood lipid,glycosylated hemoglobin,understanding of diabetic peripheral neuropathy(DPN)disease of TCM syndrome factor distribution characteristics and the level of performance,combined with the feature of TCM syndrome factor discussed DPN disease syndrome factor and the correlation of the lower extremities vascular lesions.MethodsAccording to the diagnostic criteria,inclusion and exclusion criteria of traditional Chinese and western medicine,349 patients diagnosed with DPN in our hospital from January 2016 to January 2020 were reviewed.According to the proportion of the same symptoms and signs in different syndrome elements in syndrome differentiation,the four diagnostic contents were quantified and scored,and the total score of each syndrome element was obtained,and the score ≥70 syndrome elements were included in the study.The distribution of each syndrome element was calculated and the total integral of each syndrome element was compared.The differences of ABI,lower limb artery color doppler ultrasound score,blood lipid and glycosylated hemoglobin were compared among groups.According to the results,the Chinese medicine mechanism was used to elucidate the distribution law of the syndrome elements of xiaoshi bi and the overall etiology and pathogenesis of its correlation with the degree of vascular diseases.Results1.349 cases of DPN were collected,and the main characteristics of the four diagnosis were summarized as follows:thirst induced drinking in 241 cases,accounting for 69.1%,and dry mouth in 237 cases,accounting for 67.9%.There were 222 cases of limb numbness,accounting for 63.6;Night urinary frequency more than 134 cases,accounting for 38.4%;The frequency of urination was more than 115 cases,accounting for 33.0%.Limb pain in 111 cases(31.8%);98 cases of god’s fatigue and idleness;Sixty-four cases(28.1%)and fu Yang pulse disappeared or weakened,accounting for 18.4%.Blurred vision in 39 cases,accounting for 11.2%;There were 37 cases of foot coldness,accounting for 10.6%.Tongue veins were mainly:tongue dark in 196 cases,accounting for 56.2%;Light in 154 cases,accounting for 44.1%;104 cases(29.8%)in Hong Kong;Light red in 60 cases,accounting for 17.2%;There were 199 cases of moss-thin white,accounting for 57.2%.Greasy(thick)78 cases,accounting for 22.3%;Moss yellow 31 cases,accounted for 8.9%.There were 155 cases,accounting for 44.4%.140 cases,accounting for 40.1%;106 cases,accounting for 30.4%;There were 67 cases of pulse-slip,accounting for 19.2%.2.There were 296 cases of above DPN disease whose exemplification score was ≥70 points.There were 8 syndromes:dry 210 cases(70.9%);There were 206 cases of Yin deficiency,accounting for 69.6%.Blood stasis in 59 cases(19.9%);Eighty-four patients(28.4%)with Yang deficiency.There were 54 cases of qi deficiency,accounting for 18.2%.Blood deficiency in 57 cases(19.3%);There were 20 cases of sputum,accounting for 6.8%.Fever in 18 cases(6.1%).3.Among the 296 cases of DPN,30 cases(10.1%)were purely positive.There were 62 cases(20.9%)of pure deficiency syndrome.There were 204 cases,accounting for 68.9%.There were 63 cases of single syndrome type(21.3%).There were 109 cases with two syndromes,accounting for 36.8%.There were 81 cases with three syndromes,accounting for 27.4%.There were 34 cases with four syndromes,accounting for 11.5%.There were 8 cases with five syndromes,accounting for 2.7%.One case of six syndromes accounted for 0.3%.Among the complex syndrome elements(2 syndrome elements as above):21 cases of pure deficiency,accounting for 7.1%;There were 8 pure cases,accounting for 2.7%.There were 204 cases,accounting for 68.9%.4.7 kinds of main syndrome factor two groups of patients with integral comparison,Yin deficiency is higher than the debility,dryness,blood deficiency,blood stasis(Mann-Whitney U=2315.00,5483.00,2792.500,3396.500,2520.00,P=0.000<0.05);Drying is lower than the qi deficiency,blood deficiency,Yang deficiency,blood stasis,phlegm-heat(Mann-Whitney U=3284.000,1181.5000,1952.5000,2640.000,1547.000,P=0.000<0.05);Blood and qi deficiency were lower than Yang deficiency(Mann-Whitney U=1397.5000,1161.5000,P=0.000<0.05).There are significant differences.5.There was no significant difference in the total lower limb arterial color doppler ultrasound score and the ABI value of tibial foot of both lower limbs in the 6 main syndromes(except phlegm and heat)group(Kruskal-Wallisχ2=4.282,1.128,4.664,0.623,2.615,P=0.510,0.952,0.458,0.987,0.759>0.05).Conclusion1.To summarize the data of the four diagnosis,"excessive thirst,excessive urine and dry mouth" was the most common,followed by numbness and pain in the limbs.The tongue is light and dark,and the pulse is fine.In the analysis of DPN syndrome elements,≥ 70 minutes of syndrome elements were dryness,Yin deficiency,Yang deficiency,blood stasis,blood deficiency,qi deficiency,phlegm and heat.Yin deficiency and Yang deficiency were the most common,followed by Yang deficiency,with less blood stasis and least phlegm and heat.Simple empirical evidence is the least,most of the patients presented the composite syndrome type with the inclusion of virtual and solid mainly,and Yin deficiency and dryness mainly.2.The score of Yin deficiency syndrome element is higher than that of qi deficiency,blood stasis,dryness and blood deficiency,,phlegm-heat.Although the dryness is the most,its integral value is lower than other syndromes,and the expression degree of syndromes is weak.The phlegm-heat frequency was the least,but there was no significant difference between the score and other syndromes in each group except Yin deficiency,indicating that the degree of performance was not low.The frequency of blood stasis is less,but the feature of blood stasis syndrome element of tongue darkening appears most frequently in patients,indicating that blood stasis may be more insidious.There was no significant difference between the integral of Yang deficiency and Yin deficiency,and the diagnosis degree of Yin and Yang deficiency was similar,both of which were more obvious than qi deficiency and blood deficiency.3.There was no significant difference in the correlation between qi-deficiency,blood-deficiency,yin-deficiency,yang-deficiency,blood-stasis and dryness syndrome factors and the total color doppler ultrasound score and ABI value of lower limb artery in the 6 syndrome factors group. |
