| ObjectiveTo explore the correlation between cerebral blood flow and the occurrence of intracerebral microbleeds,and to analyze the distribution characteristics of TCM yin-yang syndrome types in CMBS patients by analyzing the characteristics of cerebral blood flow detected by transcranial Doppler(TCD)in patients with CMBs,which is helpful for further clinical study of CMBs and can provide evidence-based basis for the diagnosis of patients with CBMs and help to provide more scientific diagnosis and treatment for patients with it.MethodsAccording to the inclusion criteria,115 out patients and inpatients of Shenzhen Hospital of traditional Chinese medicine who were diagnosed as cerebral micro hemorrhage by magnetic resonance functional imaging between April 2017 and November 2019 were selected,and TCD examination was improved.115 patients who had no cerebral micro hemorrhage detected by MRI and who also improved TCD were selected as control group.In addition to collecting the general baseline information such as the patient’s name,gender,age,outpatient registration number/inpatient number,time of treatment,personal history such as smoking and drinking,as well as the past medical history of hypertension,diabetes,hyperlipidemia,serious arrhythmia hyperhomocysteinemia,cerebral infarction,cerebral hemorrhage,the medical records mainly collected include Including:1.The four diagnostic data,such as symptoms and signs,tongue and pulse,obtained from TCM observation,listening and questioning,are used to divide Yin and Yang syndrome types.2.Collect the peak systolic blood flow velocity,average blood flow velocity and diastolic blood flow velocity of bilateral anterior cerebral artery,middle cerebral artery and posterior cerebral artery measured by TCD,and divide the blood flow velocity into slowing down,normal and increasing according to the diagnostic criteria.3.Collect the imaging data of CMBs patients,and grade the cerebral micro hemorrhage according to the diagnostic criteria.input the statistical software spss25.0 for statistical analysis,and analyze the statistical results.To To compare the differences of general baseline data and risk factors between CMBS and non CMBS groups,to compare the differences of TCM syndrome type distribution and cerebral blood flow characteristics between CMBS and non CMBS groups,to put the classified variables into binary regression equation model and gradually eliminate them,and finally to get binary logistic regression equation and corresponding statistics of CMBS.To compare the distribution characteristics of different types of CMBS in different grades and groups of cerebral blood flow diagnosis.Results1.There was no significant difference in age,gender,smoking history,drinking history,diabetes history,hyperlipidemia history,serious arrhythmia history,tumor history,cerebrovascular tumor history and cerebral hemorrhage history between CMBS group and non CMBS group(P>0.05),but there was significant difference in cerebral infarction history and hypertension composition between the two groups(P<0.05).2.The velocity of cerebral blood flow in CMBS group was significantly lower than that in non CMBS group(P<0.05).3.There was significant difference between the diagnosis group of cerebral blood flow and the classification of cerebral micro hemorrhage between yin and Yang Syndrome Types(P<0.05).4.In this study,abnormal cerebral blood flow velocity(OR=14.654,95%CI=7.603-28.242)and ischemic factors(OR=2.248,95%CI=1.165-4.336)were independent risk factors for CMBs.Conclusion1.The blood flow velocity of CMBS group was lower than that of non CMBS group,in which RPCA(VS),LPCA(VS),rmca(VS),rmca(VM),LMCA(VS),LMCA(VM),Raca(VS),LACA(VS)were more obvious,and the decrease amplitude of middle cerebral artery and anterior cerebral artery was higher than that of posterior cerebral artery.2.CBF abnormality and ischemic factors are independent risk factors for CMBS.Patients with CBF abnormality and ischemic factors are prone to cerebral microhemorrhage.3.In CMBS group,the main syndrome type was Yang partial deficiency,accounting for 74.8%of the total,while in non CMBS group,the main syndrome type was Yin deficiency,accounting for 63.5%of the total.Different from traditional cerebral hemorrhage,the main pathogenesis of CMBS is Yang deficiency.4.The diagnosis of CBF in Yang partial deficiency syndrome is mainly slow down,accounting for 64.1%of the total number of the group,while in Yin partial deficiency,the diagnosis of CBF is mainly normal,accounting for 79.4%of the total number of the group,and the blood flow speed in Yang partial deficiency group is slower than that in CBF.5.The CMBS in the Yang partial deficiency group were mainly classified as grade 4,accounting for 30.2%of the total number of the group,and grade 3-4 accounted for 51.1%of the total number of the group.The CMBS in the Yin partial deficiency group were mainly classified as grade 1,accounting for 51.7%of the total number of the group,grade 3-4 only accounted for 17.2%of the total number of the group.The CMBS in the Yang partial deficiency group were higher than those in the non CMBS group. |
