Study On The Service Mode Of Community Home-based Care For The Elderly In The View Of The Medical-nursing Combined Care In Tianjin City

With the increasing aging of the population in China,the demand for pension services is also increasing.Family care and institutional care have their own advantages,but also have their own shortcomings.Because of the advantages of both,community home-based care has gradually been recognized by the elderly,and has become a national advocated way of providing for the aged.With the increase of age,the physical function of the elderly is declining.Health problems need to be paid more attention.Therefore,it is an effective way to actively respond to the trend of population aging to develop the integrated medical and nursing services at the community level.The service mode of community home-based care for the elderly in the view of the medical-nursing combined care(hereafter this text will be abbreviated as the mode of integrated medical and nursing services for the aged in the community)is a very valuable research direction.As one of the cities with serious aging in China,Tianjin began to explore the service mode of community home-based care for the elderly at an early stage,and has achieved some results in the pilot work of integrated medical and nursing services for the aged in the community,which is of great research value.This paper mainly adopts the methods of documentation,interview and case study to research the mode of integrated medical and nursing services for the aged in the community in Tianjin city.This article combs the previous literature,defines the concept of the combination of community home-based care for the elderly and the medical-nursing combined care,and establishes an analytical framework guided by healthy aging theory,social support network theory and social embedding theory.On the basis of generalizing and summarizing the development process and the current situation of the community home-based pension service in Tianjin city,this text chooses two streets of Tianjin as typical cases to analyze the implementation status of their service modes.This paper summarizes the applicable conditions and advantages of the two modes,discusses the reasons for their shortcomings.Based on the successful experience at home and abroad and the actual situation of Tianjin,this paper puts forward some countermeasures and suggestions from six aspects:establishing and perfecting the supervision and management system,broadening the sources of funds,attaching importance to the training of service personnel,scientifically planning community day care service outlets and service objects,effectively allocating community health service resources,and actively promoting informal support services.This paper provides reference and help for the improvement and optimization of the integrated medical and nursing services for the aged in the community in Tianjin city.
