| Objiective: With the development of economy and the increase of life expectancy,the incidence of gynecological malignant tumors has increased significantly,especially Endometrial cancer(EC).As we all know,the incidence of cervical cancer is high in the past.With the promotion of the third-stage screening model for cervical cancer,the mortality rate of cervical cancer has decreased significantly.The incidence of endometrial cancer has been significantly higher than that of cervical cancer since 2011,according to the Beijing cancer registration office.Risk factors for endometrial cancer include obesity,hypertension and diabetes.According to relevant scholars,the incidence of hypertension in high economic provinces has increased 。The incidence of diabetes in China is also increasing.the main screening method is staged curettage,The steps are complex and take too long,the cervix needs to be dilated during the operation,There are also many risks of curettage,such as uterine perforation,peritoneal implantation of endometrial cancer cells,increased recurrence rate of endometrial cancer,uterine adhesion and massive bleeding,ascending infection,and water poisoning.it difficult to perform an effective screening test(mass screening)for endometrial cancer.With the continuous expansion of the high-risk population of endometrial cancer,it is imperative to conduct an effective endometrial cancer survey,and more effective pathological sampling and diagnosis methods are urgently needed to make the general population more capable of the survey.Early prevention;Early detection,;Early diagnosis and Early treatment of endometrial cancer can improve the prognosis of endometrial cancer.With the development of diagnostic methods for endometrial cytologic test(ECT),endometrial cytologic test has emerged.ECT is a kind of endometrial sampling device(ESD)that can be used to directly obtain the endometrial cytological specimens.In addition,it can also be combined with liquid-based thin-layer cytology paper technology for cytological examination.In the process of ESD sampling,anesthesia and dilation of the cervix are not required,pain is reduced,discomfort of patients is significantly reduced,and bleeding and adverse events during anesthesia are reduced.Endometrial atrophy also occurs in postmenopausal patients and is not affected by the thickness of the endometrium.Disposable plastic equipment materials,eliminate the adverse infection between patients,less volume,suitable for the size of the cervix.To avoid the occurrence of cervical laceration and uterine perforation,the equipment is simple and easy to operate,which can be completed by single operation in the outpatient department.There is no need for hospitalization,which saves a lot of manpower and time,and large population screening can be carried out within a limited time.At the same time,few researchers have studied the sensitivity and specificity of the Pipelle tubes and the Pipelle brush in diagnostic curettage.The aim of this study is to detect endometrial carcinoma using the Pipelle suction brush and Tao brush.Methods : Randomized control method,A total of 200 patients who needed curettage and were willing to accept endometrial cytology examination in the outpatient department and inpatient department of shenzhen second people’s hospital from October 2016 to October 2017 were collected.Divided into two groups by random number table method.Tao Brush endometrium sampler set,Pipelle endometrium sampler set.Sensitivity,specificity,sampling satisfaction,pain score(VAS),blood loss,and patient compliance of the two groups were collected,respectively.Results: The Pipelle endometrial sampler group and Tao Brush endometrial sampler group had no significant difference in blood loss,pain,complications or compliance(P >0.05).When compared with the Pipelle and Tao Brush endometrial sampler respectively,the coincidence rate,sensitivity,specificity,positive predictive value and negative predictive value of endometrial curettage were 98%,96.7% and 96.5%,100% and 100%,respectively.Under the control of related factors,there is no statistical difference in the endometrial cytology compliance rate between the Tao Brush endometrial sampler group and the Pipelle endometrial sampler group(P >0.05).Conclusions:(1)both Tao Brush endometrial sampler and Pipelle endometrial sampler can be used to detect endometrial cancer in high-risk population at early stage.(2)At the time of screening endometrial cancer,there was no significant difference in sensitivity and specificity between the Tao Brush endometrial sampler and the Pipelle endometrial sampler.(3)At cost,the Tao Brush endometrium sampler is superior to the Pipelle endometrium sampler at best... |
