Clinical Observation On The Therapeutic Effect Of Shengyang Sanhuo Decoction On Cancerous Fever Of Stage Ⅱ-Ⅲ Primary Hepatocellular Carcinoma (Spleen Deficiency And Yang Stagnation Type)

Objective:By observing the antipyretic effect,the onset time of antipyretic effect,the repeated times of body temperature within 5 days and the cumulative time of fever,the integral of TCM symptoms and KPS score,the clinical effect of Shengyang Sanhuo Decoction on primary hepatocellular carcinoma fever in stage II-III was discussed,which provided a new basis for the treatment of primary hepatocellular carcinoma fever in stage II-III with Shengyang Sanhuo Decoction.In order to provide experience accumulation,open up ideas and better guide clinical application for further in-depth and systematic research.Methods:From April 2018 to February 2019,70 cases of primary hepatocellular carcinoma(PHC)with cancer fever of spleen deficiency and Yang stagnation type were diagnosed in the Oncology Department of Henan Traditional Chinese Medicine Hospital.Adopt random number table method,according to the principle of 1:1 distribution,divided into two groups includes treatment group(35 cases)and control group(35 cases).Naproxen tablets in control group,and Shengyang Sanhuo Decoction in treatment group.The two groups were treated for 5 days.The antipyretic effect,onset time of antipyretic effect,repeated body temperature and cumulative fever time in 5 days,TCM symptom score and KPS score were observed to compare the clinical effects of the two groups.Results:1.General data comparison:After x~2 test and t test analysis,the two groups of patients’gender,age,body temperature before treatment and KPS score before treatment(all P>0.05),no statistical difference,comparability.2.Comparison of antipyretic effect,laboratory index and integral of TCM syndromes.(1)Comparison of antipyretic effect:the total effective rate of treatment group was 94.3%,while that of the other group was 62.9%,with statistical difference(P=0.001),the antipyretic effect of the treatment group was better than that of the control group.(2)Comparison of antipyretic onset time:The onset time of antipyretic therapy between the two groups has statistical difference(P=0.000).The onset time of antipyretic therapy in the control group was significantly shorter than that in the treatment group.(3)Comparing of temperature repetition times in 5 days:Compared to the control group,the frequency of temperature repetition in the treatment group was more less,and the recurrence rate was more superior with significant difference(P=0.000).(4)Comparing the cumulative fever time,the treatment group was shorter,as well as in the antipyretic effect(P=0.010).(5)KPS score comparison after treatment:24 cases in the treatment group were markedly effective,the total effective rate was 98.6%;15cases in the control group were markedly effective,the total effective rate was 92.9%.The marked efficiency of KPS was significantly higher in the treatment group with significant difference(P=0.021).(6)Laboratory examination comparison:There was no statistical difference between the two groups before and after treatment(P>0.05).There were significant differences in laboratory examination between the treatment group and the control group(P<0.05).There were significant differences in CRP and PCT between two groups(P<0.05),while WBC was no significant difference(P>0.05).The treatment group was significantly surperior in WBC,CRP and PCT values,while the control group could only improve CRP and PCT values.(7)Comparisons of TCM syndrome scores:TCM syndrome scores of the two groups after treatment were improved(P=0.000),but the improvement of TCM syndrome scores was more significant in the treatment group(P=0.000).(8)Comparisons of symptom integrals of TCM syndromes:There was statistical difference in syndrome integrals between the treatment group and the control group(P<0.05);there was no statistical significance in the other symptoms except hypochondriac pain,bumps,stupor,la ck of food,fever,dizziness,tinnitus(P>0.05),except hypochondriac pain,mucus,irritability,jaundice,fever,dizziness and tinnitus(P<0.05).The treatment group can significantly improve the symptoms from a sharp contrast with the control group.The scores of symptoms before treatment in the two groups except hypochondriac pain,chest tightness,excessive rest,ruff,depression,dark complexion,knee weakness and hemorrhage had no statistical difference(P<0.05).After treatment,the scores of symptom s between the two groups except hypochondriac pain and knee weakness had no statistical difference(P>0.05),and there was statistical difference in the other symptoms(P<0.05).Conclusion:Shengyang Sanhuo Decoction has a longer antipyretic effect than naproxen tablets.However,Shengyang Sanhuo Decoction can improve the symptoms of primary hepatocellular carcinoma in stage II-III,prolong the action time,reduce the number of repeated fever and cumulative fever time,reduce the recurrence rate,improve the physical condition of patients,and improve the compliance of patients with traditional Chinese medicine.
