Study On Risk Factors And Traditional Chinese Medicine Syndrome Elements Of Delirium Syndrome In The Elderly

Objective:To review the clinical data of delirium syndrome in the elderly,analyze the distribution of TCM clinical syndromes and related factors in this disease,to deeply study the Chinese medicine’s understanding of its etiology and pathogenesis,and to guide the traditional Chinese medicine treatment of delirium syndrome in the elderlyMethods:This subject retrospectively studied the medical records of 84 patients with delirium syndrome who were hospitalized in the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from January 1,2014 to December 1,2019.Collect the patient’s age,gender,past history,evoking factors,four TCM diagnosis data,etc.After entering the data into the database for analysis,explore the distribution of senile delirium syndrome syndromes and risk factors.Under the careful guidance of the tutor guidance group,the clinical characteristics,etiology and pathogenesis,syndrome differentiation,and prognosis of the elderly delirium syndrome were summarized.Results:A total of 84 elderly patients with delirium syndrome were collected in this study.The average age is 81.71 years.The brain(mind)is the most common pathological syndrome,accounting for 78.6%,and the proportion of each pathological factor is brain(mind)78.6%>heart 33.3%>kidney 17.9%>spleen 16.7%>liver 11.9%.The proportion of disease-related syndromes is yin deficiency 40.5%>Qi deficiency 18.9%>Yang deficiency 31%>Blood deficiency 27.4%>Sputum 21.4%>Fire 16.7%>Blood stasis 13.1%>Closed 10.7%>Blood heat 8.3%>Qi Lags 4.8%.The most common predisposing factors in this study were 35 cases of infection(41.7%),32 cases of surgery(38.1%),and 30 cases of cerebrovascular disease(35.7%).Operation is closely related to brain(mind and spirit),infection is closely related to syndrome elements of heat,blood heat,yin deficiency and yang deficiency,and heart failure is closely related to syndrome elements of heart and Qi deficiency.Conclusion:The main disease of delirium syndrome in the elderly is in the brain(heart and mind),which is closely related to heart,kidney,spleen and liver;the disease is mainly yin deficiency,qi deficiency,yang deficiency,blood deficiency,sputum,fire,blood stasis,closed,Blood heat and stagnation.Patients with heart failure often have Qi deficiency and disease location in the heart.The main pathogenesis is:brain dysfunction,brain spinal insufficiency,and atrophy.In the treatment of senile delirium syndrome,it is necessary to distinguish the false and the real samples according to the syndrome differentiation,and the basic treatment principle is to support the healthy and eliminate pathogenic factors.Prevention is the most effective strategy to reduce the occurrence and development of delirium and its adverse consequences.We should evaluate the risk factors of delirium in early stage,control the basic diseases of patients actively,use the psychoactive drugs that may induce delirium cautiously,prevent the occurrence of infection and acute cardiovascular and cerebrovascular diseases,maintain the electrolyte balance,and prevent the environment from causing malignant stimulation to patients.If delirium has occurred,it should be found and intervened early.
