Objective:A systematic study of reproductive endocrine,glycolipid me tabolism,controlled ovarian hyperstimulation outcome and differential ex pression of serum,follicular fluid miRNA-146a in patients with the phleg m dampness and liver depression polycystic ovary syndrome(PCOS).Analysis of the formation mechanism of the phlegm dampness syndrome and liver dep ression syndrome in polycystic ovary syndrome,providing theoretical basi s for the diagnosis and treatment of PCOS.Methods:The study selected 60 patients who met the inclusion criteri a within Vitro-fertilization-embryo transfer(IVF-ET)of the Reproductive and Genetic Center in the Affiliated Hospital of the Shandong University of TCM,including phlegm dampness PCOS group,liver depression PCOS grou p and normal control group 20 cases each.Serum and follicular fluid samp les and their clinical data,reproductive hormones and biochemical indexe s were collected from all enrolled patients.Finally,selected 18 cases o f phlegm dampness PCOS group,16 cases of liver depression PCOS group and 14 cases of the normal control group to detect the expression level of m iRNA-146a in serum and follicular fluid of PCOS patients by fluorescence quantitative PCR.It was using SPSS21.0 software to count sample data and further analysis.Results:1.The BMI and WHR of patients with phlegm dampness PCOS gro up were higher than those of liver depression PCOS group and normal contr ol group,the difference was significant(P<0.05).2.Compared with the no rmal control group,increased level of LH、T、HOMA-IR、TC、TG、LDL-C and decreased level of HDL-C in the phlegm-dampness PCOS group,with a signif icant difference(P<0.05);Phlegm dampness PCOS group compared with liver depression PCOS group,the T、HOMA-IR、TG、TC、LDL-C level increased,the LH、HDL-C level decreased,with a significant difference(P<0.05);The LH level in the liver depression PCOS group was significantly higher than th at of the normal control group,and the difference was statistically sign ificant(P<0.05).3.The expression levels of serum miRNA-146a were signif icantly different among the groups.Compared with the normal control grou p,the expression levels of miRNA-146a in the serum of patients with phle gm dampness PCOS group and liver depression PCOS group were higher(P<0.05),and the expression level of miRNA-146a in phlegm dampness PCOS group was higher than that of liver depression PCOS group(P<0.05).4.The expre ssion level of miRNA-146a in follicular fluid was significantly different among the groups.The expression level of miRNA-146a in the follicular f luid of phlegm dampness PCOS group and liver depression PCOS group was to wer than that of the normal control group(P<0.05).The expression level o f miRNA-146a in the follicular fluid of phlegm dampness PCOS group was lo wer than that of the liver depression PCOS group(P<0.05).Conclusion:1.Patients with phlegm dampness polycystic ovary syndrom e are more prone to alodominal obesity,hyperandrogenism and glycolipid me tabolism disorders such as insulin resistance,while the outcome of contr olled ovarian hyperstimulation is general.2.Patients with liver depress ion polycystic ovary syndrome often have elevated luteinizing hormone,wh ich is more closely related to the dysfunction of the hypothalamus-pituit ary-ovary(HPO)axis.3.Serum miRNA-146a is highly expressed specifically in PCOS,especially in patients with phlegm dampness PCOS,which is clos ely related to disturbance of secretion in the body and abnormal metaboli sm of glucose and lipid.4.Follicular fluid miRNA-146a is specifically l ow expression in phlegm dampness PCOS,which is closely related to follic ular dysplasia,oocyte quality and embryo quality in patients with phlegm dampness PCOS.
