| Objective: The clinical features,treatment and prognosis of 12 cases of Plasmoblastic lymphoma(PBL)in our hospital were studied,and the data of 142 cases of PBL patients in China were retrospectively analyzed to explore the clinical features and prognosis of PBL patients in China.Methods: Data of 12 PBL patients treated in the first affiliated hospital of hebei north university from January 2015 to January 2020 were collected,and 142 PBL patients whose full text was published on zhiwang,wanfang and baidu academic databases were sorted out.The clinical,pathological features and prognosis were analyzed retrospectively.Results:1.There were 12 hiv-negative PBL patients in our hospital,including 7 males and 5 females.The ratio of males to females was 1.40:1,and the median age of onset was 59(39-71)years.Database data 142 patients with PBL,including 102 males and 40 females,had a male-female ratio of 2.55:1.HIV positive patients accounted for 3.2%,with a median age of 55(10-92)years.2.In 12 patients with PBL,there were 3 cases(3/12)in the primary site of the stomach,2 cases(2/12)in the chest wall,2 cases(2/12)in the lymph nodes,and 2 cases(2/12)in the lungs.Among them 9 cases(75.0%)for Ⅲ or Ⅳ period,accompanied by B symptoms in 4 cases(33.3%),5 cases(41.7%)patients with bone marrow infiltration;Database of 142 patients with PBL primary site lymph node 31 cases(21.8%),27 cases of gastrointestinal tract(19.0%),18 cases(14.8%),oral,nasal 21 cases(12.7%),etc.,Ⅲ or Ⅳ period patients accounted for 69.5%.Among them,38.9% of the patients had B symptoms and 33.8% had bone marrow infiltration.3.All the patients in our hospital showed typical PBL morphological characteristics.Among the 12 PBL patients in our hospital,7 were CD20 positive,9 were CD38 positive,2 were CD138 positive,and 4 were mum-1 positive.1 EBER was positive,1 EBER was negative,and the rest were not tested.Database data of PBL patients with tumor cell immunophenotypes: CD20,CD38,CD138 and mmum-1 accounted for 14.1%,89.7%,80.5% and 79.4%,respectively.EBER has a positive rate of 46.5%.4.Among the 12 patients with PBL in our hospital,2 patients did not receive treatment after diagnosis,and 1 patient lost follow-up immediately after diagnosis without treatment.The remaining patients were treated with CHOP±E(cyclophosphamide + doxorubicin + vinblastine + prednisone + etoposide)chemotherapy.3 patients received surgery + chemotherapy,1 patient received chemotherapy + radiotherapy,and 6 patients received chemotherapy alone.The longest follow-up period was 48 months,the shortest was 2 months,and the overall effective rate was 90% after initial treatment.5.In this study,single factor analysis was used to conclude that clinical stage,B symptoms and EBV infection were the prognostic factors affecting the overall survival of PBL patients(P < 0.05).Multivariate analysis showed that clinical stage was an independent factor of poor prognosis(P < 0.05).Conclusions:1.The vast majority of PBL patients in China are HIV negative;When the doctor in Ⅲ or phase Ⅳ;The common lesion sites were gastrointestinal tract and nasal cavity.2.EBV infection rate is slightly lower than that of foreign patients..3.The CD20 expression rate of immunophenotype in PBL patients was slightly higher than that in foreign patients.4.Clinical stage,B symptoms and EBV infection may affect the prognosis of PBL patients.5.Clinical staging is an independent prognostic factor for PBL. |
