Clinical Application And Training Effect Analysis Of Vocal Cord Leukoplakia Narrow Band Imaging Endoscopic Diagnosis And Classification

[OBJECTIVE]To explore the clinical value of narrow band imaging(NBI)endoscopic diagnostic classification and related content training in identifying the benign and malignant vocal cord leukoplakia.[Methods]Thirty cases of vocal cord leukoplakia were selected to conduct a questionnaire survey on 20 professional laryngologists.The benign and malignant vocal cord leukoplakia were assessed in white light imaging(WLI)mode,and then a special training on vocal cord leukoplakia NBI laryngoscope diagnosis was conducted.Then,combined with the NBI model to assess the benign and malignant vocal cord leukoplakia,calculate the assessment of benign and malignant vocal cord leukoplakia by WLI,NBI before training,and NBI after training,compare the difference between the groups using x2 test,and Kappa test to evaluate the consistency of diagnosis.[Results]The accuracy of WLI,NBI(before training)and NBI(after training)diagnosis of vocal cord leukoplakia was 63.3%,69.8%,and 77.0%,respectively.The training of NBI endoscopy has a significant effect on improving the accuracy of laryngologist assessment,of which NBI endoscopic training is most significant in improving the specificity of the vocal cord leukoplakia differential diagnosis.The consistency of diagnosis of vocal cord leukoplakia in WLI mode is weak,and the consistency of diagnosis of vocal cord leukoplakia after NBI training is improved compared with that before training.Twenty laryngoscope doctors had poor or weak consistency in the diagnosis of vocal cord leukoplakia NBI classification,and improved the consistency of merging vocal leukoplakia types Ⅰ and Ⅱ and Ⅳ,Ⅴ,and Ⅵ to general.[Conclusion]The training course of NBI endoscope can improve the differential diagnosis ability of laryngoscopy for vocal cord leukoplakia.The new NBI endoscopic diagnosis and classification needs to be further simplified in order to facilitate clinical application.
