| Objective: To explore the correlation between cerebral microbleeds and blood pressure variability in patients with silent lacunar infarction.Methods: Patients with asymptomatic lacunar infarction admitted to the Department of Neurology and Geriatrics of the First Affiliated Hospital of Hebei North University from October 2018 to December 2019 were collected,and 50 patients with cerebral SWI suggestion and cerebral microbleeds were selected as the study group.In the same period,50 patients without cerebral microbleeds were selected as the control group.Record detailed clinical data of two groups of patients,measure biochemical indicators after admission,calculate and record blood pressure variability related parameters after monitoring 24-hour ambulatory blood pressure,and type according to blood pressure circadian rhythm: normal rhythm(dippers),abnormal rhythm(non dipper,anti dipper,super dipper),graded according to the number of cerebral microbleeds lesions(Level 1:1-3,Level 2: 4-10,Level 3:> 10).Results: Among the parameters related to ambulatory blood pressure in the study group,the 24-hour systolic blood pressure(24h SBP),24-hour diastolic blood pressure(24h DBP),night systolic blood pressure(NSBP),Night diastolic blood pressure(NDBP),and 24 hour systolic blood pressure-coefficient of variationt(24h SBP-CV)and 24 hour diastolic blood pressure--coefficient of variation(24h DBP-CV)were higher than those in the control group,and the differences were statistically significant(P<0.05).In the study group,those with an abnormal rhythm of ambulatory blood pressure were higher than those in the control group,and the difference was statistically significant(P <0.05).Among the three different types of abnormal rhythms,the incidence of cerebral microbleeds in the non-dippers rhythm was relatively high,and the difference was statistical significance(P <0.05).There was a positive correlation between 24 h SBPCV and 24 h DBP-CV and the severity of cerebral microbleeds(r = 0.561,r= 0.435,respectively,P <0.05).Conclusion: Studies have confirmed that for asymptomatic patients with lacunar infarction,When the mean level of blood pressure increased and the variability increased,cerebral microbleeds were more likely to occur.Abnormal circadian blood pressure rhythm is an important influencing factor of cerebral microbleeds,and the influence of anti dipper is more significant.The coefficient of variation of dynamic blood pressure is positively related to the severity of cerebral microbleeds. |
