Symptom Clusters Investigation And The Influencing Factors In Patients With Neurogenic Bladder

Objectives:Investigate the symptoms of patients with neurogenic bladder and explore the types of symptom clusters in patients with neurogenic bladder.To analyze the influencing factors of symptom clusters in patients with patients neurogenic bladder and help patients reduce the burden of symptoms and improve their quality of life.Methods:From September 2020 to March 2021,150 patients with neurogenic bladder from five three Grade A hospital,three second Grade A hospitals in Jinan City were studied,and Neurogenic Bladder Symptom Score,2-item patient Health Questionnaire,Generalize Anxiety Disorder Scale,Neurogenic Bowel Dysfunction Score,Brief Fatigue Inventory and Verbal Rating Scale were used to evaluate the symptoms of patients with neurogenic bladder.Methods of exploratory factor analysis were used to explore the types and composition of symptom clusters in patients with neurogenic bladder.The questionnaires were used to investigate the factors influencing the symptom clusters in patients with neurogenic bladder.The questionnaire included demographic data,physical health data and disease and treatment related data in patients with neurogenic bladder,a single factor analysis and a multivariate linear regression analysis was carried out to select the factors influencing the symptom clusters of patients with neurogenic bladder.Results:1.The analysis of symptom in patients with neurogenic bladder showed:the higher incidence of symptom was loss of libido(81.3%),nocturia(75.3%),urinary incontinence(70.0%),pain(57.3%),fatigue(53.3%),bowel dysfunction(50.7%),micturition arduous(30.7%),urine rheological fine(27.3%),bladder filling(26.6%),urgency(26.0%),anxiety(26.0%),depression(25.3%),sleep disorders(22.0%).Through exploratory factor analysis,4 types of symptom clusters were found,which were paruria related symptom cluster,emotion related symptom cluster,pelvic floor dysfunction related symptom cluster and pain related symptom cluster.2.The results of single factor analysis of symptom clusters in patients with neurogenic bladder showed:education,family monthly income per preson,disease causes,course of the disease ADL and micturition way were related to the severity scores of paruria related symptom cluster,the differences were statistically significant(P<0.05);gender was related to the severity scores of emotion related symptom cluster,the differences were statistically significant(P<0.05);ADL was related to the severity scores of pelvic floor dysfunction related symptom cluster,the differences were statistically significant(P<0.05);medical expenses payment mode was related to the severity scores of pain related symptom cluster,the differences were statistically significant(P<0.05).3.The results of multifactor analysis of symptom clusters in patients with neurogenic bladder showed:the influencing factors of paruria related symptom cluster were gender,family monthly income per preson and micturition way.The influencing factors of emotion symptom cluster were gender.The influencing factors of pelvic floor dysfunction related symptom cluster were gender,course of the disease and ADL.The influencing factors of pain symptom cluster were family monthly income per preson.Conclusions:1.Patients with neurogenic patients have multiple symptoms at the same time and the incidence of symptoms is high,especially the symptoms such as loss of libid,nocturia,urinary incontinence,pain,fatigue and bowel dysfunction.It is suggested that the medical staff should comprehensively evaluate the physical and mental symptoms of patients with neurogenic patients and effective intervention measures should be taken in time to reduce the burden of symptoms of patients.2.There are 4 types of symptom clusters in patients with neurogenic bladder,namely paruria related symptom cluster,emotion related symptom cluster,pelvic floor dysfunction related dysfunction related symptom cluster and pain related symptom cluster,which can provide reference for the future description of the symptom clusters in patients with neurogenic bladder,for the early early intervention of symptoms to relive symptom burden and help to improve the quality of life.3.The results of multivariate linear regression analysis of symptom clusters in patients with neurogenic bladder showed:the influencing factors of paruria related symptom cluster were gender,family monthly income per preson and micturition way.The influencing factors of emotion symptom cluster were gender.The influencing factors of pelvic floor dysfunction related symptom cluster were gender,course of the disease and ADL.The influencing factors of pain related symptom cluster were family monthly income per preson.These results can establish early prediction of symptoms and provide scientific evidence for the intervention of symptom clusters.
