To Discuss The Prevention Strategy Of Cerebral Small Vessel Disease From The Correlation Between Cerebral Small Vessel Disease And Constitution Types Of Traditional Chinese Medicine

Objective:Through traditional Chinese medicine(TCM)constitution identification and related investigations of patients with cerebral small vessel disease to understand the distribution of TCM constitution.On the basis of integrating the concept of TCM treatment for disease prevention and the method of body differentiation and adjustment,this paper is to explore suitable and feasible management methods for the prevention and treatment of cerebral small vessel disease with integrated Chinese and Western medicine.Methods:During the period November 2018 to October 2020,230 outpatients and inpatients with cerebral small vessel disease admitted to the Department of Neurology in our hospital.They were willing to cooperate in completing the four diagnoses of traditional Chinese medicine and TCM constitution identification.The data of 230 patients with cerebral small vessel disease were obtained and analyzed by SPSS 22.0 statistical software.To study the distribution characteristics of TCM constitution type of cerebral small vessel disease and the correlation between the subtypes of cerebral small vessel disease and constitution type of TCM.Results:1.Among the 230 patients with cerebral small vessel disease,there were 90males(39.1%)and 140 females(60.9%),with a male-to-female ratio of 1:1.56.The age range of the patients was 41～87 years old,with an average age of 60.81±9.43 years old.73.9% of the patients were 50～ 69 years old,which indicating that most of them in the middle and elderly stage.The BMI of 57.0% of the patients was 24.0 or above,which indicating that more than half of them were overweight or obese.2.230 patients with cerebral small vessel disease included 100 cases of lacunar infarction combined with leukoaraiosis,84 cases of simple lacunar infarction,and 46 cases of simple leukoaraiosis.Determination of TCM constitution type of 230 patients: 44 cases of neutral constitution,71 cases of Yang-deficiency constitution,35 cases of blood-stasis constitution,29 cases of Yin-deficiency constitution,23 cases of phlegm-dampness constitution,11 cases of Qi-deficiency constitution,8 cases of Qi-stagnation constitution,8cases of damp-heat constitution,1 case of special constitution.3.There were statistically significant differences in the contrast of neutral constitution,Yang-deficiency constitution and Qi-stagnation constitution with gender(P<0.05).Among them,neutral constitution was dominant in males,while Yang-deficiency constitution and Qi-stagnation constitution was dominant in females.There were statistically significant differences in the contrast of phlegm-dampness constitution,Qi-deficiency constitution and blood-stasis constitution with age group(P<0.05),all of whom were 60 ～ 69 years old.There were statistically significant differences in the contrast of Yang-deficiency constitution with BMI(P<0.05),Yang-deficiency constitution was dominant in overweight.4.Lacunar infarction:On the distribution of TCM constitution in lacunar Infarction of different severity grades,there were statistically significant differences in the contrast of Yang-deficiency constitution,blood-stasis constitution with lacunar cerebral infarction(P<0.05).Among them,blood-stasis constitution was the main type of lacunar infarction in grade 1(5.4%),and Yang-deficiency constitution was the main type of lacunar infarction of grade 2 and 3,which were 13.6% and 16.3% respectively.On the comparison of risk factors of cerebrovascular disease in different severity grades of lacunar Infarction,there were statistically significant differences in the contrast of the severity grades of lacunar infarction with age,smoking,and alcohol consumption(P<0.05).The type of TCM constitution and the risk factors of cerebrovascular disease were defined as independent variables,and lacunar infarction was defined as dependent variable.The single factor analysis showed that the main risk factors of lacunar infarction included age,smoking,drinking alcohol,Yang-deficiency constitution,and blood-stasis constitution.The severity grades of lacunar infarction gradually increased with age.5.Leukoaraiosis:On the distribution of TCM constitution in leukoaraiosis of different severity grades,there were statistically significant differences in the contrast of phlegm-dampness constitution,blood-stasis constitution with leukoaraiosis(P<0.05).Among them,blood-stasis constitution was dominant in leukoaraiosis of grade 1 and2,respectively 8.9%,5.5%.Phlegm-dampness constitution was the main type in leukoaraiosis of grade 3,which was 5.5%.On the comparison of risk factors of cerebrovascular in different severity grades of leukoaraiosis,there were statistically significant differences in the contrast of the severity grades of leukoaraiosis with age,hypertension and smoking(P<0.05).The type of TCM constitution and the risk factors of cerebrovascular disease were defined as independent variables,and leukoaraiosis was defined as dependent variable.The single factor analysis showed that the main risk factors of leukoaraiosis included age,hypertension,smoking,phlegm-dampness constitution,and blood-stasis constitution.The severity grades of leukoaraiosis gradually increased with age.Conclusion:1.The patients with cerebral small vascular disease were 50～69 years old,which indicating that most of them in the middle and elderly stage.2.In the biased constitution,patients with cerebral vascular disease were mainly composed of Yang-deficiency constitution,blood-stasis constitution and phlegm-dampness constitution.3.Neutral constitution was dominant in males,while Yang-deficiency constitution and Qi-stagnation constitution was dominant in females.4.Lacunar infarction:The severity grades of lacunar infarction is related with the constitution of TCM,and blood-stasis constitution was the main type of lacunar infarction in grade 1,and Yang-deficiency constitution was the main type of lacunar infarction of grade 2 and 3.The severity grades of lacunar infarction is related with age,smoking,alcohol consumption.Therefore,the main risk factors of lacunar infarction included age,smoking,drinking alcohol,Yang-deficiency constitution,blood-stasis constitution.5.Leukoaraiosis:The severity grades of leukoaraiosis is related with the constitution of TCM.Blood-stasis constitution was dominant in leukoaraiosis of grade 1 and 2,and phlegm-dampness constitution was the main type in leukoaraiosis of grade 3.The severity grades of leukoaraiosis is related to age,hypertension,smoking.Therefore,the main risk factors of leukoaraiosis included age,hypertension,smoking,phlegm-dampness constitution,blood-stasis constitution.6.The severity grades of lacunar infarction and leukoaraiosis gradually increased with age.
