Clinical Study On The Treatment Of Gouty Arthritis With Dampness And Heat Accumulation By Reducing Acid And Reducing Lipids

Objective: The purpose of this project is to compare the clinical efficacy of Jiangan jiangjiangzhi cream and relying on kauxi tablet alone,so as to observe and analyze whether the treatment of jiangacidjiangzhi cream combined with relying on corxi tablet in treating patients with damp and hot accumulation gout arthritis is effective and safe.It can provide the basis for the wide application of Jiangan Jiangzhi ointment and Kaoxi tablet in the treatment of rheumatic and heat accumulation gouty arthritis,and provide new ideas and new methods for the diagnosis and treatment of the rheumatic and heat accumulation gouty arthritis.Methods: From November 2019 to November 2020,72 patients in the rheumatology clinic were selected as the research objects,and the syndrome differentiation was damp heat accumulation type.They were randomly divided into treatment group and control group with 36 cases in each group.The control group was treated with etoscoxib,while the treatment group was treated with jiangsuanjiazhi ointment on the basis of the control group.There was no significant difference in the baseline data(gender,age,course of disease,TCM syndrome score)between the two groups(P > 0.05).The changes of VAS score,ESR value,CRP value,TCM syndrome efficacy,TCM symptoms(joint color red,joint swelling,joint fever,joint pain,poor flexion and extension,thirst,urine yellow)score and blood uric acid value before and after treatment were compared between the two groups.Results:1.There was no significant difference in gender,age,course of disease and TCM syndrome score(P > 0.05).2.VAS score comparison before and after treatment: the P values of normality and homogeneity of variance test were greater than 0.05.After different treatments,the VAS scores of the two groups decreased,and the degree of decline was more significant than before treatment,indicating that the treatment group can significantly relieve the pain of patients with gouty arthritis of damp heat accumulation type.The treatment group was better than the control group in improving pain;3.Comparison of ESR and CRP values before and after treatment: P values were greater than 0.05 by the test of normality and homogeneity of variance.After different treatments,ESR and CRP values of the two groups decreased significantly,indicating that the treatment group can significantly relieve ESR and CRP values of patients with gouty arthritis of damp heat accumulation type.The treatment group was better than the control group in improving the laboratory indexes;4.Comparison of TCM syndromes: the effective rate of the treatment group was86.11%.The effective rate of the control group was 55.56%.There was significant difference between the treatment group and the control group(P < 0.01).5.Single symptom score comparison: by rank sum test,after treatment,the treatment group was better than the control group in the treatment of joint skin color,joint swelling,joint fever,joint pain,poor flexion and extension,thirst and other symptoms(P < 0.05),and the effect of the two groups was similar(P > 0.05).6.Comparison of blood uric acid before and after treatment: comparison within the group,the blood uric acid value of the control group at 1 week,2 weeks and 4 weeks after treatment compared with that before treatment,after paired t test,P > 0.05,the difference was not statistically significant.The blood uric acid value of the treatment group at 2weeks and 4 weeks compared with that before treatment,P < 0.05.The difference was statistically significant.There was no significant difference between the treatment group and the control group in the blood uric acid value before and after 1 week of treatment,P >0.05 by independent sample t-test,but the blood uric acid value after 2 weeks and 4 weeks of treatment was p < 0.05.The difference was statistically significant.Conclusion:1.The clinical effect of jiangsuanjianzhi ointment combined with etoscoxib in the treatment of gouty arthritis(damp heat accumulation type)is significantly better than that of the control group,which can effectively relieve the clinical symptoms of patients with gouty arthritis.2.Jiangsuanjianzhi ointment combined with etoscoxib is safe and effective in the treatment of gouty arthritis of damp heat accumulation type,which can be widely used in clinical treatment.
