BackgroundA rhegmatogenous retinal detachment(RRD)occurs when a tear in the retina leads to fluid accumulation with a separation of the neurosensory retina from the underlying retinal pigment epithelium, which is the most normal type of retinal detachment. There are many studies show that the incidence rate of RRD grows from 6.3/100000 to 17.9/100000.Rhegmatogenous retinal detachment not only seriously affects the visual quality of affected eye,but also has a risk of retinal tears or retinal detachment on the other eyes. Therefore, for an ophthalmologist,it is highly important for observing contralateral eye in eraly clinicals in the course of treating patients with rhegmatogenous retinal detachment.ObjectiveTo observe the peripheral retinal vascular fluorescence leakage of fundus fluorescence angiography in contralateral eye on patients with unilateral RRD and analyze its possible causes and the relationship with age, peripheral retinal degeneration, diopter, and gender.MethodsProspective study.Observation group:114 patients with unilateral rhegmatogenous retinal detachment and ages from 13 to 73,average age(45.58±14.87)years old were collected from The First Affiliated Hospital of Zhengzhou University during October 2018 to August 2019.Control group:68 patients with unilateral idiopathic macular epiretinal membrane(IMEM)who were admitted at the same time. There were 30 males(30 eyes)and 38 females(38 eyes),the age ranged from 35 to 71 years, with the mean age of(56±10.52)years old. After admission, all patients undergo routine optometry, intraocular pressure, slit lamp microscope, binocular OCT,IOL-master,binocular fundus panretinoscopy, binocular FFA,etc.after exclusion of contraindications.Focus on the observation of the fluorescent leakage of the retinal blood vessels around the contralateral eye and analyze the possible causes and the relationship with age, diopter and gender.Results1.There were 25 unilateral RRD patients found with fluorescence leakage of peripheral retinal vessels in contralateral eye in total of 114 cases. The incidence rate is 21.93%,of which nine cases of(36.00%)fluorescence leakage area involves one quadrant,16 cases of(64.00%)fluorescence leakage area involve two and more quadrants.2.There were 25(21.93%)unilateral RRD patients with fluorescence leakage of peripheral retinal vessels in contralateral eye and 4(5.88%)unilateral IMEM patients with fluorescence leakage of peripheral retinal vessels in contralateral eye.The difference was statistically significant(χ2=8.189,P<0.05).3.There were 11(44%)unilateral RRD patients with Fluorescence leakage of peripheral retinal vessels in the contralateral eye had peripheral retinal degeneration in the leakage area,19(21.35%)unilateral RRD patients without fluorescence leakage of peripheral retinal vessels in the contralateral eye had peripheral retinal degeneration. The difference was statistically significant(χ2=5.165,P<0.05).4.The incidence rate of fluorescence leakage of peripheral retinal vessels in contralateral eye on patients with unilateral RRD decreases with the age.The incidence rate is 80%on patients<18 years old,25.64%from 18-45 years old,17.31%from 46-60 years old,and 11.11%on patients>60 years old.The incidence rate of patients≤45 years old is higher than that of patients>45 years old,and the difference was statistically significant(χ2=4.092,P<0.05).5.The incidence rate of fluorescence leakage of peripheral retinal vessels in contralateral eye on patients with unilateral RRD is increased with the increase of diopter,4(10.53%)in group A,3(17.65%)in group B,7(25.00%)in group C and 11(35.48%)in group D.The incidence rate of fluorescence leakage of peripheral retinal vessels in contralateral eye on patients with unilateral RRD in high myopia group(35.48%)is higher than non-high myopia group(16.87%),and the difference was statistically significant(χ2=4.569,P<0.05).6.The incidence rate of fluorescence leakage of peripheral retinal vessels in contralateral eye on patients with unilateral RRD with different gender were 13(18.06%)in male,12(28.57%)in female, and there was no significant difference(χ2=1.713,P>0.05).Conclusion1.Some patients with unilateral rhegmatogenous retinal detachment have fluorescence leakage of peripheral retinal vessels in the contralateral eye,and the incidence is higher than that the patients with idiopathic macular epiretinal membrane.2.The incidence rate of fluorescence leakage of peripheral retinal vessels in contralateral eye on patients with unilateral RRD decreases with age, increases with increased diopter.3.The incidence of peripheral retinal degeneration was increased on contralateral eye in patients with unilateral rhegmatogenous retinal detachment and fluorescence leakage of peripheral retinal vessels.
