ObjectiveCarrying out the analysis of the current situation of the configuration of portable color ultrasound equipment of the county medical community,environmental analysis,and the research of the site selection theory,build the county medical alliance medical equipment configuration’s site selection model,and conduct empirical research to provide decision-making basis for optimizing the medical equipment configuration of the county medical alliance.MethodsCollected and analyzed the current situation of the county medical alliance’s portable color Doppler ultrasound configuration through field surveys and general descriptive statistical methods,and combined relevant literature data to carry out the county medical alliance’s medical equipment configuration environment analysis using the PEST analysis method;based on the theory of value engineering,carry out the theoretical basic research and model construction of the portable color ultrasound configuration of the county medical alliance;select the representative county medical alliance as the empirical case,obtain the empirical data corresponding to the model parameters through the field survey method,and use the genetic algorithm to carry out configuration’s site selection results calculation.ResultsThe analysis results of the current configuration showed that the institutional allocation rate of portable color Doppler ultrasound in the survey area is only 21.6%,the average number of units deployed in the hospital is 0.25,the average number of qualified personnel in the hospital is 0.6,and the domestic brands account for71.7%;The analysis results of the configuration environment showed that under the condition of rapid progress in the construction of the county medical alliance,there is a lack of overall medical equipment configuration standards for the county medical alliance,and there is an urgent need for scientific configuration and standardization of medical equipment resources;medical institutions have "check dependence" and medical equipment configuration lacks standardization;site selection technology has been widely used in power engineering,logistics,transportation and other fields,but there are few applications in the medical and health field.The results of theoretical analysis showed that based on the theory of value engineering,the core influencing factors of the current theoretical model of medical equipment allocation of the county medical alliance mainly include: cost and function.The function is based on the residents’ medical expenses(the sum of commuting time and inspection waiting time).The results of the model construction for the site selection showed that taking portable color Doppler ultrasound as an example,starting from the perspective of medical institutions and patients,comprehensively considering influencing factors including distance,time,cost,personnel qualifications and other constraints,a method for selecting medical care was constructed.The mathematical model of the organization’s configuration.The empirical research results showed that based on the established constraints,after 5000 iterations of the calculation algorithm is terminated,the model outputs 5relatively optimized configuration institutions among the 9 matching institutions,and when the number of configuration institutions for each configuration point is 1,the total value of the configuration in the medical alliance reaches the maximum.Conclusion1.The deployment rate of portable color Doppler ultrasound in the primary medical institutions of the county medical alliance is low,and it does not meet the relevant national equipment configuration standards,which affects the development and improvement of primary health service capabilities.2.Scientific selection and allocation of medical equipment is an important means to promote the construction of medical alliance with high quality;3.This research is based on the theory of value engineering,and the proposed portable color Doppler ultrasound equipment configuration selection model is scientifically constructed,and the calculation results are more consistent with actual needs,and have the characteristics of objectivity,standardization and reliability;4.The research on configuration and selection will help enrich and perfect the connotation of the county medical alliance construction and realize the overall development.It is an important reference for promoting the high-quality construction and efficient operation of the county medical alliance in our country. |
