ObjectiveTo explore the clinical application value of mixed reality technology in the treatment of complex kidney stones with PCNL,and to improve the theoretical basis for the treatment of complex kidney stones with PCNL.MethodsFrom January 2019 to December 2020,40 patients who were diagnosed with complicated kidney stones and percutaneous nephrolithotomy(PCNL)in the Department of Urology,First People’s Hospital of Yunnan Province,were randomly divided into mixed reality(MR)group and control group each had 20 cases.All patients underwent CT urography(CTU)before the operation.The MR group saved the original imaging data in DICM format and imported it into a special software system(Materialise Mimics system + MR 3D system)to establish a holographic 3D image model.It is used for preoperative communication between doctors and patients,formulating surgical plans and intraoperative guidance.The control group used CTU images to communicate with doctors and patients before surgery,determine surgery plans,and provide intraoperative guidance.TThe t-test and chi-square test were used to statistically analyze the intraoperative puncture time,operation time,number of puncture needles,puncture renal calyx coincidence,puncture depth,hemoglobin reduction,stone clearance rate,postoperative complications and so on.ResultsAll 40 patients were successfully operated without death.All patients in the MR group successfully reconstructed the three-dimensional model.The preoperative general data and demographic characteristics of the two groups of patients were not significantly different.In terms of doctor-patient communication,the MR group’s understanding of their own disease degree,surgical methods,surgical risks,and satisfaction scores were significantly higher than those of the control group(P<0.05).In comparison of intraoperative conditions,the MR group was significantly better than the control group in puncture time,renal calyx compliance,and intraoperative blood loss(P<0.05).The difference between the expected puncture depth and the actual puncture depth in the MR group was not significant,The predicted puncture depth in the control group was less than the actual puncture depth(P<0.05).Conclusion1.The use of mixed reality holograms can be well used for doctor-patient communication,can strengthen the patient’s understanding of the disease itself,surgical methods and surgical risks,increase the trust between doctors and patients,and build a good doctor-patient relationship.2.Based on the personalized 3D model of mixed reality technology,the preoperative puncture channel design is carried out to improve the accuracy of puncture;at the same time,the preoperative simulated puncture reduces the PCNL learning curve.3.During the operation,mixed reality technology can clearly see the relationship between kidneys,stones,and blood vessels,helping the surgeon to accurately locate,establish the best channel,reduce surgical complications,and improve the rate of stone removal.
