| Objective:To observe the clinical efficacy of acupoint injection of compound Angelica injection combined with needle knife in the treatment of lumbar three transverse process syndrome of qi stagnation and blood stasis,and comparing with simple needle knife therapy,the efficacy and safety of compound angelica injection in relieving pain and reducing recurrence rate were discussed,to provide effective guidance for clinical practice in the future.Methods:The cases in this study were collected from 66 patients with TLTPS who met the inclusion criteria diagnosed and treated by the outpatient and inpatient departments of Traditional Chinese Medicine Kunming Hospital from November 2019 to December 2020.The patients were randomly divided into observation group(n=33)and control group(n=33),in the whole process of the study,there were 3 cases in each group,and finally 60cases were observed.Treatment plan:the control group was only treated with needle knife.The observation group was treated with acupoint injection of compound Angelica injection combined with needle knife,the acupoints were selected as L1-L3Jiaji acupoint and Ashi acupoint.10 ml of compound Angelica injection 4 ml,lidocaine injection 4 ml and 0.9%sodium chloride injection 2 ml were extracted.1 ml/was injected into L1-L3Jiaji acupoint on the affected side,and 5 ml was injected at Ashi acupoint.First,3 ml solution was injected into the tip of L3transverse process in different directions.Then 1 ml solution was injected into deep fascia and superficial fascia respectively.Needle knife treatment was performed after acupoint injection.Both groups were treated once a week,and the total course of treatment was twice.The VAS score and ODI score of the patients were statistically analyzed before treatment,after the first treatment,after the second treatment and 2 months after all treatment(follow-up).The recurrence of the disease was statistically analyzed at the second month of follow-up,and the therapeutic effects of the two groups were compared.Results:1.There was no significant difference between the two groups in terms of sex,age,course of disease,VAS scores and ODI scores before treatment(P>0.05).2.VAS scores:Intra-group comparison:the VAS scores of the two groups showed a downward trend in the corresponding time period after treatment,and the test results were statistically significant between the two groups at different time periods after treatment(P<0.05).Comparison between groups:there was no significant difference in VAS scores between the two groups after the first treatment(P>0.05),but there was significant difference in VAS scores after the second treatment and the second month after all treatments(P<0.05).3.ODI scores:Intra-group comparison:the ODI scores of the two groups showed a downward trend in the corresponding time period after treatment,and the test results were statistically significant between the two groups at different time periods after treatment(P<0.05).Comparison between groups:there was no significant difference in ODI scores between the two groups after the first treatment(P>0.05),but there was significant difference in ODI scores between the two groups after the second treatment and the second month after all treatments(P<0.05).4.The total clinical efficacy of the two groups:In the observation group,11 cases(36.67%)were cured,17 cases were improved(56.67%),2 cases were not cured(6.67%),and the total effective rate was 93.33%.In the control group,6 cases were cured(20.00%),19cases improved(63.33%),2 cases were not cured(16.67%),the effective rate was 83.33%.According to statistical analysis,there was no significant difference in effective rate between the two groups(P>0.05).5.The number of disease recurrence after 2 months follow-up:the number of recurrent cases in the acupoint injection combined with needle knife group was less than that in the needle knife treatment group(P<0.05).Conclusion:1.Acupoint injection combined with needle-knife therapy and simple needle-knife therapy can relieve pain,improve functional activity and improve quality of life in TLTPS patients with qi stagnation and blood stasis.2.Acupoint injection combined with needle knife is effective,and compared with simple needle knife therapy,it can reduce the recurrence rate.3.Acupoint injection combined with needle knife in the treatment of qi stagnation and blood stasis TLTPS has no adverse reactions,which is safe and effective,and is worthy of clinical promotion. |
