| [Objective] : In this study,I used "Group Psychotherapy" and " Solution Focused Approach " to intervene the psychological disorders of appropriate-age and elderly pregnant women in the third trimester of pregnancy and compared the difference in intervention effects of appropriate-age and elderly pregnant women with psychological disorders in the third trimester of pregnancy. This study will provide scientific basis for perinatal mental health monitoring of appropriate-age and elderly pregnant women during perinatal period.[Methods]:From November 1,2019 to October 31,2020,the research objects that meet the inclusion criteria(28-30 weeks of gestation;SAS and SDS double positive;appropriate age: ≥25 and <35 years old, elderly: ≥35 years old)were selected using convenient sampling method in the obstetrics department of our hospital. The appropriate-age and elderly pregnant women were randomly divided into observation group A,observation group B and control group,6 groups in total, with 40 cases in each group(after excluding lost follow-up).After grouping, the intervention was performed in different ways. The observation group A was treated by Group Psychotherapy and routine health education during pregnancy;the observation group B was intervened by Solution Focused Approach and routine health education during pregnancy;and the control group only received routine health education during pregnancy. The intervention sites were mainly obstetric clinics, ward, fetal heart care units and We Chat visits. Each intervention time was greater than 30 minutes, with a total of 7 interventions were from the 30 weeks of pregnancy to 6 weeks after delivery.Measuring tools include pregnant woman’s general information questionnaire,Self-rating Anxiety Scale and Self-rating Depression Scale. The data were analyzed and compared using the chi-square test(Fisher exact probability method was used when the expected frequency was less than5),Kruskal-Wallis test,one-way ANOVA(LSD post-hoc comparison was used for statistical differences),and independent sample T-test,respectively.When P < 0.05,the differences were considered statistically significant.[Results]:1.In 28-30 weeks of pregnancy:there was no significant difference(P > 0.05)in anxiety and depression scale scores between the three groups of appropriate-age pregnant women, indicating that the three groups are comparable. The results of the elderly group were consistent with those of the appropriate-age group. 2.At 3 days and 42 days after delivery :The scores of anxiety and depression scale of appropriate-age pregnant women were statistical differences(P < 0.05)between the three groups. There was no statistical difference(P > 0.05)in the scores of anxiety and depression scale between observation group A and observation group B,and the scores of anxiety and depression scale in the observation groups were significantly lower than those in the control group. The incidence of anxiety and depression in the observation group A and observation group B are similar or the same and are significantly lower than the control group. The results of the elderly group were consistent with those of the appropriate-age group.3.Comparing the scores of anxiety and depression scale between the three groups of appropriate-age pregnant women at 28-30 weeks of pregnancy,3 days after delivery and42 days after delivery, the differences are statistically significant(P < 0.05).The three groups had different intervention methods, but the scores of anxiety and depression scale, and the incidence of anxiety and depression showed a decreasing trend with the extension of intervention time and the increase of intervention times. The results of the elderly group were consistent with those of the appropriate-age group. 4.At 3 days and 42 days after delivery: There were significant differences(P < 0.05)in the scores of anxiety and depression scale between the Group Psychotherapy observation group(the observation group A of appropriate-age pregnant women, the observation group A of elderly pregnant women)and the Solution Focused Approach observation group(the observation group B of appropriate-age pregnant women, the observation group B of elderly pregnant women),and the scores in the appropriate-age group were significantly lower than those in the elderly group.[Conclusion]:1.There is no statistical difference in the intervention effect of Group Psychotherapy and Solution Focused Approach on psychological disorders of appropriate-age and elderly pregnant women, which are superior to conventional health education during pregnancy. 2.Group Psychotherapy and Solution Focused Approach can significantly reduce the level of psychological disorders in appropriate-age and elderly pregnant women at 3days and 42 days after delivery, and the level of psychological disorders decreases significantly with the extension of intervention time and the increase of intervention times. 3.The intervention effect of Group Psychotherapy and Solution Focused Approach on psychological disorders of appropriate-age pregnant women is better than that of elderly pregnant women. |
