Clinical Study Of Modified Sang Bai Pi Decoction In The Treatment Of Senile AECOPD And Its Effect On Inflammatory Mediators

Objective: To research the clinical efficacy of modified Sang bai pi Decoction in the treatment of elderly patients with AECOPD(phlegm heat stagnation of lung syndrome)and its effect on inflammatory mediators,also to evaluate the therapeutic impact of modified Sangbaipi Decoction on patients with COPD’s attack,enrich a certain objective basis for clinical treatments.Methods: Firstly,72 patients from the Department of geriatrics and Department of pulmonary disease of traditional Chinese medicine,which met the inclusion criteria would be taken into account.They were randomly divided into one group(treatment group)and the other group(contorled group),and each group had 36 cases.The control group was given routine western medicine treatment,and the treatment group was given modified Sangbaipi Decoction on the basis of the control group.The treatment cycle of the two groups was 2 weeks.Then I observed and recorded the two groups’ clinical symptoms、physical sign score,、Serum IL-6、Serum TNF-α、 lung function、chest CT and security index.Finally,the datas were received and analyzed by statistical methods.Result:67 patients were accomplished the study,the treatment group had 34 cases and33 cases were in the matched group.(1)Comparison of clinical efficacy:The total effective rates of each group were91.18% and 72.73% respectively,and the treatment group had better effect(P < 0.05).(2)Comparison of total scores of clinical symptoms and sign scores:Both groups were treated,the fractions of the two groups decreased,which shows a statistical difference(P < 0.05),and the therapeutic effect of the treatment group was better than that of the control group(P < 0.05).(3)Comparison of inflammatory indexes:The levels of IL-6 and TNF-α decreased in both groups(P < 0.05),and the treatment group was superior to the matched group.(4)There was a negative correlation between inflammatory index and FEV1%(P <0.05),but a positive correlation with the severity of pulmonary function(P < 0.05).(5)Comparison of the total effective rate of chest CT : The treatment group was64.7%,while the matched one was 33.3%.Though both group could improve situation of Pulmonary inflammation,the treatment group overmatched(P < 0.05).Conclusions:1.The clinical symptoms could significantly improved elderly patients with AECOPD applying to modifying Sang ba ipi decoction.2.Adding Sang bai pi decoction could reduce the levels of serum IL-6 and TNF-α so much that it could clear the inflammatory medium to some extent,which could ease the symptom of lung inflammation effectively.3.Inflammatory indexes could estimate the severity degrees of disease,so as to be controlled as soon as possible.
