| Objective:To investigate the application value of high frequency ultrasound combined with shear wave elastography in acute gastrocnemius injury.Methods:A total of 34 patients with acute unilateral gastrocnemius injury in our hospital from November 2019 to December 2020 were selected,including 23 males and 11 females,aged 14~68 years old,with an average age of 48.29±12.80 years old.In the healthy control group,there were 30 patients,including 19 males and 11 females,aged 17~66 years old,with an average age of 41.40±11.89 years old.The general information was compared between the injury group and the healthy control group.Patients in the injury group were divided into group A(complete rupture)with 8 cases,group B(partial rupture)with 10 cases,group C(small tear)with 16 cases.Using high frequency ultrasound,respectively,to the distal,proximal of injured and injured sites structure form and the echo profile of injured site observation,then using shear wave elastography technology(shear wave elastography,SWE)measurements of gastrocnemius injury young’s modulus and shear wave velocity,shear wave velocity,SWV),compared with healthy controls.Meanwhile,the Young’s modulus and SWV of the gastrocnemius muscle in the same position of the affected side and the healthy side of the same patient were compared and analyzed.The ROC curve was drawn to calculate the optimal truncation value of SWE in the diagnosis of acute gastrocnemius injury,and to evaluate the diagnostic value of SWE in the diagnosis of acute gastrocnemius injury.Results:Compared with the healthy control group,there was statistically significant difference in age between the injury group and the healthy control group(P<0.05),but there was no statistically significant difference in gender between the injury group and the left and right sides(P>0.05).Compared with the healthy control group,the maximum Young’s modulus and SWVmax of the injury group(A,B and C groups)were decreased,and the differences were statistically significant(P<0.05).The maximum Young’s modulus and SWVmax of the gastrocnemius muscle on the affected side were decreased in the injured group(A,B and C groups)compared with the gastrocnemius muscle at the same position on the healthy side,and the difference was statistically significant(P<0.05).In the diagnosis of acute gastrocnemius injury by shear wave elastography,the best cutoff value of the maximum Young’s modulus was <12.3k Pa,the sensitivity was 88.24%,and the specificity was 97.06%.The optimal cut-off value of SWVmax was <2.5m/s,the sensitivity was 94.12%,and the specificity was 94.12%.It shows that this method has a high diagnostic value in the diagnosis of acute gastrocnemius injury.Conclusion:High frequency ultrasound combined with shear wave elastography can better display the location,scope and structural morphology of the injured gastrocnemius muscle,and can quantify the degree of stiffness and softenness of the injured gastrocnemius muscle,which has a higher diagnostic value in acute gastrocnemius muscle injury,and can also provide an important reference value for clinical treatment and prognosis. |
