A Study Of Clear Aligner Treatment Technique On The Realization Rate Of Maxillary Expansion In Adult Patients

Objectives: To study clear aligner treatment technique on the realization rate of maxillary expansion in adult patients by measuring unilateral expansion amount and the effect of different preset expansion amounts on the maxillary arch expansion realization rate.Materials and methods: Based on the inclusive standards,20 patients were selected from the patients who were treated in the orthodontic department of Dalian Stomatological Hospital Affiliated to Dalian Medical University from September 2018 to December 2020,which contained 7 males and 13 females,aged 18 to 35 years old(26.05±5.10 years old),All patients were treated with clear aligner treatment technique.This study does not use any auxiliaries other than attachments and the study looked only at the first round of aligners,excluing refinement.According to the unilateral expansion amount of maxillary canine,maxillary first premolar,maxillary second premolar and maxillary first molar preset in Clin Check,all samples were divided into 3 groups,unilateral preset expansion amount of 0.5-1 mm is group A,unilateral preset expansion amount of 1-2 mm is group B,unilateral preset expansion amount of 2-3 mm is group C,each group includes maxillary canine,maxillary first premolar,maxillary second premolar and maxillary first molar.Aquare three-dimensional maxillary occlusal digital models before and after treatment,use Geomagic Wrap 2017 software to perform overlap and measurement analysis,and calculate actual unilateral expansion amount of maxillary canine,maxillary first premolar,maxillary second premolar and maxillary first molar.Calculate the maxillary arch expansion realization rate by comparing with unilateral preset expansion amounnt of the corresponding tooth in Clin Check.Group the data and analyze the effect of different preset expansion amounts on the maxillary arch expansion realization rate.SPSS 25.0 statistical analysis software is used to process and analyze the measurement data,and the number of maxillary teeth in ecah group is analyzed by chi-square test to exclude the influence of sample size on experimental results.The difference between preset expansion amount and actual expansion amount is statistically analyzed by paired t test.The effect of different preset expansion amounts on the maxillary arch expansion realization rate is perform by One-way ANOVA.P<0.05 indicates that the difference is statistically significant.Results: 1.There are statistically significant difference between preset and actual expansion amount of maxillary canine,maxillary first premolar,maxillary second premolar and maxillary first molar in each group(P<0.05).2.Average arch expansion realization rates of maxillary canine,maxillary first premolar,maxillary second premolar and maxillary first molar in group A are respectively 79.57% 、 76.36% 、72.53% 、 63.45%.Average arch expansion realization rates of maxillary canine,maxillary first premolar,maxillary second premolar and maxillary first molar in group B are respectively 78.51% 、 74.84% 、 70.80% 、 60.18%.Average arch expansion realization rates of maxillary canine,maxillary first premolar,maxillary second premolar and maxillary first molar in group C are respectively 78.06% 、 73.36% 、69.11%、58.25%.3.There is no statistically significant difference in arch expansion realization rate of maxillary canine among different preset arch expansion groups(P>0.05).There is statistically significant difference in arch expansion realization rate of maxillary first premolar,maxillary second premolar and maxillary first molar among different preset arch expansion groups(P<0.05).Conclusion: When adults use clear aligner treatment technique for maxillary exxpansion,there is significant difference between preset and actual arch expansion amount.In different preset arch expansion groups,the arch expansion realization rate decreases gradually from maxillary canine,maxillary first premolar,maxillary second premolar to maxillary first molar,it is more necessary to increse the preset expansion amount when expanding the maxillary posterior area in clinical practice.There is statistically significant difference in arch expansion realization rate of maxillary first premolar,maxillary second premolar and maxillary first molar among different preset arch expansion groups,and the arch expansion realization rate decreases as the amount of preset arch expansion increase.
