Research purposes:To explore the impact of cervical conization and different types of conization(LEEP,CKC)on pregnancy and outcome,and provide references for women who wish to give birth after cervical conization and clinical medical workers for the management of female pregnancy after conization.Research method:The clinical data of patients admitted to the obstetrics department of our hospital from January 2016 to September 2020 were collected,who had undergone cervical conization(including LEEP and CKC)for high-grade cervical intraepithelial lesions(CINII-III).Case group of this study: A total of 124 patients who met the inclusion criteria were collected,including 63 cases in the LEEP group and 61 cases in the CKC group.The control group of this study: randomized the clinical data of the same proportion of normal pregnant women.Obtain effective information by consulting the electronic case and telephone follow-up,including the general condition of the patient(age,height,weight gain during pregnancy,weight before pregnancy,number of pregnancy,parity,smoking and drinking history,etc.),cervical conization type(LEEP,CKC),The interval between surgery and pregnancy(≤12 months,>12months),pregnancy outcome(premature delivery,miscarriage,full-term delivery),whether cervical cerclage was performed during pregnancy,andwhether there was a premature rupture of membranes and other complications during prehnancy,,gestational week of delivery,newborn condition(birth weight,low birth weight,Apgar score(1 minute,5minutes)),ect.To study the influence of different cervical conization procedures and the influence of the interval between operation and pregnancy on the outcome of pregnancyand to study whether preventive cervical cerclage is beneficial to the improvement of pregnancy outcome of patients after CKC surgery.Research result:1.Comparison between the normal group and the conization groupThere was no statistically significant difference between the normal group and the conization group in general conditions(P>0.05).The difference between the two groups was statistically significant in terms of premature delivery rate,premature rupture of membranesand birth weight of newborns(P<0.05).2.Comparison between the normal group and the LEEP groupThere was no statistically significant difference between the normal group and the LEEP group in general conditions(P>0.05).There was no significant difference between the two groups in terms of premature delivery rate,premature rupture of membranes,miscarriage rate,newborn birth weight,low birth weight infants,and neonatal Apgar scores(P>0.05).3.Comparison between normal group and CKC groupThere was no statistical difference between the normal group and the CKC group in general conditions(P>0.05).There were statistical differences between the two groups in terms of premature delivery rate,premature rupture of membranes,newborn birth weight,low birth weight infants,Apgar score,etc.(P<0.05).4.Comparison between LEEP and CKCThere was no statistical difference between LEEP group and CKC group in general conditions(P>0.05).There were statistical differences between the two groups in the rate of premature delivery and premature rupture of membranes(P<0.05).5.Different intervals from postoperative to prenancy(≤12 months,>12months)There was no significant statistical difference between the three groups in general conditions(P>0.05).There was no significant difference in pregnancy and outcome in the LEEP group(≤12 months,>12 months)(P>0.05).The preterm birth rate in the CKC group(≤12 months,>12months)was statistically different between the two groups(P<0.05).The three groups had no significant differences in newborn birth weight,low birth weight infants,and neonatal Apgar scores(P>0.05).6.Preventive cervical cerclage during pregnancy in CKC groupThere was no significant statistical difference between the case group and the control group in general conditions(P>0.05).There was a statistical difference in the abortion rate between the two groups(P<0.05).There was no statistical difference in other indicators(P>0.05).Analysis conclusion:1.Cervical conization increases the incidence of premature delivery and premature rupture of membranes in women;2.Different cervical conization procedures have different effects on pregnancy outcomes.In general,LEEP has better pregnancy outcomes than CKC,and LEEP is more appropriate for women with pregnancy needs;3.Different pregnancy intervals have an impact on the outcome of pregnancy.Women undergoing CKC surgery should appropriately extend the pregnancy interval;4.Preventive cervical cerclage did not significantly improve the pregnancy outcome of patients after CKC surgery.The pros and cons of preventive cervical cerclage after conization still need to be further explored.
