ObjectivesThis study pre-detected the levels of serum nitric oxide(NO),intercellular adhesion molecule-1(ICAM-1)and vascular cell adhesion molecule-1(VCAM-1)in children with type 1 diabetes with different course of disease,comparing their level differences to explore the changes of serum NO,ICAM-1 and VCAM-1 levels in children with T1 DM with different courses,in order to provide new ideas for the early diagnosis of diabetic microvascular complications.MethodSelected 45 children with T1 DM diagnosed for more than 1 year who were in the endocrinology department of Dalian Children’s Hospital between June 2019 and February 2021 as the case group(group A),and the case group was divided according to the length of the disease.Divided into 3 groups,15 cases in the 1-3 years(including 1year)group(group A1),15 cases in the 3-5 years(including 3 years)group(group A2),and 15 cases in the group over 5 years(including 5 years)(Group A3);In addition,30 children who received physical examination in our hospital during the same period were selected as the control group(Group B),including 15 males and 15 females,which passed the Enzyme-linked immunosorbent assay(ELISA)The serum levels of NO,ICAM-1 and VCAM-1 were detected in each case group and control group.The case group was also tested for glycosylated hemoglobin(Glycohemoglobin A1 c,HbA1c),urine microalbumin and urine β2 microglobulin.Finally,SPSS25.0 was used to statistically analyze the data,compare the level of research indicators between the case group as a whole,each subgroup of the case group and the control group,compare the level of research indicators between the subgroups of the case group,and analyze HbA1 c and urine.Correlation between microalbumin,urine β2 microglobulin and research index levels.Result1.After excluding cases with missing data,group A actually enrolled 30 cases(13boys,17 girls,average age 11.23±3.29 years),of which 15 cases were in group A1(6boys,9 girls,average age 10.53)±3.14 years old),8 cases in group A2(3 boys,5 girls,average age 10.63±2.33 years),7 people in group A3(4 boys,3 girls,average age13.43±3.95 years);Group B 30 Cases(15 boys,15 girls,average age 10.13±2.73years);the basic data such as age and gender of each group are not statistically different,and they are comparable.2.The levels of serum NO,ICAM-1 and VCAM-1 between group A and group B were statistically significant(P<0.05);the levels of serum NO and ICAM-1 in children with T1 DM were higher than those in normal children,and the serum VCAM in children with T1DM-1 level is lower than normal children.3.The differences in serum NO and ICAM-1 levels among the subgroups of group A were statistically significant(P<0.05);there was no significant difference in the levels of VCAM-1 among the subgroups of group A;further comparisons within the group were carried out,and the results showed There were significant differences in NO levels between group A2 and group B,and ICAM-1 levels between group A1 and group B(P<0.05).There was no significant difference between the other groups(P>0.05);serum NO levels were in the course of T1 DM 3-It may increase at 5 years,and the overall trend is to increase first and then decrease.Serum ICAM-1 levels increase in the course of T1 DM from 1-3 years,and the overall trend is first to decrease and then to increase.4.There was no significant correlation between the levels of NO,ICAM-1 and VCAM-1 in group A and the levels of HbA1 c,urinary microalbumin and urinary β2microglobulin(P>0.05).Conclusion1.There is a certain correlation between serum NO and ICAM-1 levels and the course of T1DM;2.The sample size of this study is small,and a larger sample,multi-center clinical study is needed to find early warning indicators of microvascular complications to guide clinical prevention,early detection,diagnosis and treatment of diabetic microvascular disease.
