Objective: To retrospectively analyze the differences in patients with early invasive breast cancer undergoing sentinel lymph node biopsy,explore the related factors and their relationships that affect the sentinel lymph node metastasis of early invasive breast cancer,and provide ideas for avoiding unnecessary sentinel lymph node biopsy for patients with early invasive breast cancer directionMethods: A retrospective analysis of the medical records of 192 patients with early invasive breast cancer who underwent sentinel lymph node biopsy from the Department of Breast Surgery,Affiliated Hospital of Jining Medical College from June 2017 to February 2020.According to postoperative paraffin pathology results,52 patients with SLN metastasis were found.In 140 cases without SLN metastasis,the basic information,imaging findings,and immunohistochemistry of the patients were statistically analyzed.Using SPSS21.0 statistical software,using statistical methods such as chi-square test and binary logistic regression to analyze the factors affecting sentinel lymph node metastasis,P<0.05 indicated that the difference was statistically significant.Results: Among the 192 early invasive breast cancer patients in this study,the average age was 51.76±10.27 years,and the average body mass index(BMI)was 25.21±3.27.The results of single factor analysis showed that there were statistical differences in the influence of risk factors such as HER-2 positive,tumor T stage,vascular tumor thrombus infiltration,and molecular typing on sentinel lymph node metastasis(P<0.05);4 molecular typing was performed The results of the pairwise comparison analysis of variance division showed that the risk of sentinel lymph node metastasis in Luminal B and HER-2overexpression breast cancer was higher than that of Luminal A,and the difference was statistically significant(P<0.05/6=0.008).There was no significant difference in the influence of patient age,BMI,tumor side,tumor histological grade,breast cancer type,ER,PR,Ki-67 and other factors on the status of sentinel lymph node metastasis(P>0.05).The results of multivariate regression analysis showed that the patient’s vascular tumor thrombus infiltration and molecular classification of breast cancer were independent risk factors for sentinel lymph node metastasis.Conclusion: 1.Early invasive breast cancer patients’ age,BMI,tumor side,tumor histological grade,breast cancer type,ER,PR,Ki-67 and other factors have no significant effect on sentinel lymph node metastasis.2.Vascular tumor thrombus infiltration in patients with early invasive breast cancer is a risk factor for sentinel lymph node metastasis.3.The molecular classification of patients with early invasive breast cancer is an influencing factor for sentinel lymph node metastasis.Luminal B and HER-2 overexpression breast cancer patients are at higher risk of sentinel lymph node metastasis than Luminal A.Other factors The impact needs to be further confirmed.
