Object:By collecting clinical data of IBS-D patients,exploring the distribution of TCM syndrome types of IBS-D.Further analysis of the correlation between IBS-D TCM syndromes and anxiety,depression,history of gastrointestinal infections,poor eating habits and other factors,provides a theoretical basis for TCM syndrome differentiation and treatment.Means:Select patients who meet the inclusion criteria from May 2020 to January 2021 in the outpatient and inpatient department of the Department of Spleen and Stomach Diseases,Nanjing Hospital of Traditional Chinese Medicine.Using SPSS24.0 software for statistical processing,statistical analysis of IBS-D patients’ anxiety and depression status,TCM syndrome distribution and the relationship between the two,and further summarize the rules.Outcome:1.TCM syndrome distribution:The composition ratio of TCM syndrome types of 114 patients with IBS-D,from high to low,was as follows:liver depression and spleen deficiency syndrome(35.1%)>spleen deficiency and dampness syndrome(30.7%)>spleen and stomach damp-heat syndrome(20.2%)>spleen and kidney yang deficiency Syndrome(7.9%)>Cold-heat miscellaneous syndrome(6.1%).2.Correlation between TCM Syndrome Types and Anxiety and Depression:The HAMD and HAMA scores of patients with liver depression and spleen deficiency syndrome were significantly higher than those with spleen and stomach damp-heat syndrome(P<0.05)and patients with spleen deficiency and dampness syndrome(P<0.05);EBS-D patients with different anxiety states have significant differences in the distribution of TCM syndrome types(P<0.05),and IBS-D patients with different depression states have no statistical significance in the distribution of TCM syndrome types(P>0.05);Logistic regression showed that IBS-D liver depression and spleen deficiency syndrome was related to anxiety.3.Correlation between TCM Syndrome Types and Other Factors:There are differences in the distribution of TCM syndrome types in IBS-D patients with different genders,HP infection status and severity of illness(P<0.05).Different age groups,smoking and drinking history,gastrointestinal infections and poor eating habits were not statistically significant in the distribution of TCM syndrome types(P>0.05).Logistic regression showed that IBS-D liver depression and spleen deficiency syndrome was related to severe disease,spleen deficiency and dampness syndrome was negatively related to severe disease,and spleen and stomach damp-heat syndrome was related to the history of HP infection.4.The relationship between IBS-D anxiety and depression and the severity of the illness:There were significantly more patients with severe illness combined with depression than moderate illness(P<0.0167).The number of patients with anxiety in moderate condition was significantly more than that in mild condition(P<0.0167),the number of patients with anxiety in severe condition was significantly more than that in mild condition(P<0.0167),and the number of patients with anxiety in moderate condition was significantly more than that in severe condition(P<0.0167).Conclusion:1.IBS-D TCM syndromes are related to anxiety and depression scores.The overall HAMD score and HAMA score of patients with liver depression and spleen deficiency syndrome are higher than those of spleen and stomach damp-heat syndrome and spleen-deficiency dampness syndrome;The distribution of TCM syndrome types of IBS-D is related to anxiety state,but not to depression state.Among them,IBS-D patients with anxiety state tend to have liver depression and spleen deficiency syndrome.2.The distribution of TCM syndrome types of IBS-D is related to gender,HP infection and severity of illness.Among them,IBS-D patients with severe disease severity tend to have the syndrome of liver depression and spleen deficiency,and rarely have the syndrome of spleen deficiency and dampness,suggesting that TCM syndrome differentiation has a certain predictive value for the severity of IBS-D.IBS-D patients with history of HP infection are prone to have spleen and stomach damp-hea syndrome.3.The anxiety and depression state of IBS-D patients is also related to the severity of the disease.Patients with anxiety and depression tend to show heavier symptoms. |
