| Objective:(1)To compare the levels of micro-inflammatory factors PTX3,IL-6 and TNF-αbetween patients with diabetic kidney disease and diabetic patients,to evaluate the value of micro-inflammatory factor PTX3 as an indicator of early diagnosis of diabetic kidney disease.(2)Evaluate Gushen Xiezhuo Heluo Concentrated Decoction’s clinical efficacy in the treatment of diabetic kidney disease,and observe its effects on renal function,proteinuria level and traditional Chinese medicine syndrome scores of patients.(3)Observe the effects of Gushen Xiezhuo Heluo Concentrated Decoction on microinflammatory factors PTX3,IL-6 and TNF-α in patients with diabetic kidney disease,evaluate the effect of improving microinflammatory state in diabetic kidney disease,and to preliminary explore the possible mechanism of action.Methods:(1)A total of 60 patients with diabetic kidney disease diagnosed in our hospital from June 2019 to December 2020 were collected,and at the same period 50 patients with type 2 diabetes with ACR<30mg/gCr were selected as the diabetic group.The blood level of PTX3,IL-6 and TNF-α of the two groups were detected and compared.(2)According to the principle of randomized control,60 diabetic nephropathy patients were divided into treatment group(n=30)and control group(n=30).The control group was given basic treatment such as hypoglycemic,blood pressure reduction,and lipid regulation,and the treatment group was given Gushen Xiezhuo Heluo Concentrated Decoction on this basis.The treatment course was 2 months.The clinical efficacy of the two groups and the changes of HbAlc、Scr、BUN、ACR、MAlb、eGFR and traditional Chinese medicine syndrome scores before and after treatment were observed.(3)Detect the changes of serum levels of PTX3,IL-6 and TNF-α in the treatment group and the control group before and after treatment,and analyze the correlation with ACR,MAlb,and eGFR.Results:(1)The levels of micro-inflammatory factors PTX3,IL-6 and TNF-α in the DKD group were significantly higher than,DM group,and the difference was statistically significant(P<0.01).(2)Control group and treatment group,the total efficiency of 60.00%and 93.33%,respectively.Treatment group was significantly higher(P<0.05).HbAlc,Scr,BUN,ACR,MAlb,eGFR and traditional Chinese medicine syndrome scores were improved after treatment in the two groups(P<0.01 or P<0.05),and the improvement was more significant in the treatment group compared with the control group(P<0.05).(3)The blood PTX3,IL-6 and TNF-α levels of the treatment group and the control group decreased after treatment,and the treatment group improved more significantly than the control group(P<0.01 or P<0.05).Correlation analysis showed that PTX3 and IL-6 were positively correlated with ACR and MAlb;and negatively correlated with eGFR;TNF-α was positively correlated with ACR and MAlb,and there was no obvious correlation with eGFR.Conclusion:(1)The micro-inflammatory factor PTX3 can be used as one of the indicator for early diagnosis of diabetic kidney disease.(2)Gushen Xiezhuo Heluo Concentrated Decoction has good clinical effects in the treatment of diabetic kidney disease,reducing the patient’s blood sugar and urine protein levels,improving renal function,and increasing the glomerular filtration rate.(3)It can reduce the levels of PTX3,IL-6 and TNF-α in patients with diabetic kidney disease,and its mechanism of action may be to inhibit inflammation,improve the micro-inflammatory state,and reduce kidney damage. |
