Retrospective Analysis Of GERD Q Score And Endoscopic Distribution In Patients With Different TCM Syndrome Types Of Gastroesophageal Reflux Disease

Objective:Explore different TCM syndrome type of gastroesophageal reflux disease GerdQ scores and distribution characteristics under endoscopy,and different TCM syndrome types of gastroesophageal reflux disease and associated risk factors such as gender,age,BMI,the connection between analysis of the possible inner link between the two,so as to provide more convenient for TCM syndrome differentiation,objective theoretical basis,so as to better play to traditional Chinese medicine in the role of the prevention and treatment of gastroesophageal reflux disease.METHODS:A retrospective study was conducted to collect 139 patients who were admitted to the Spleen and Gastric Diseases Department of Nanjing Hospital of Traditional Chinese.Medicine from January 2020 to February 2021 and who met the diagnostic criteria of traditional Chinese and western medicine for GERD.The patient information was collected through face-to-face interview and telephone return visit,and a general data questionnaire was filled in.Finally,Excel database was established,and SPSS26.0 statistical software was used for data statistical analysis.Results:(1)Distribution of TCM syndromes:Among 139 GERD patients,the syndrome types in order of proportion were:liver and stomach stagnation and heat syndrome(33.81%)>qi stagnation phlegm blocking syndrome(25.18%)>central deficiency and qi inversing syndrome(20.14%)>gallbladder heat invading stomach syndrome(10.07%)>spleen deficiency dampness heat syndrome(6.47%)>blood stasis blocking collaterals syndrome(4.32%)。(2)Analysis results of different TCM syndromes and general data:1)GERD patients with different syndromes had no statistical significance in age and BMI(P>0.05),but had differences with gender(P<0.05),and had differences with long-term smoking and drinking(P<0.05).2)There was no statistical significance in history of diabetes and hypertension among patients with different syndromes(P)0.05),but there was a difference between GERD and patients with hyperlipidemia(P=0.04<0.05).However,there was no significant difference in cholesterol,triglyceride and low density lipoprotein(P>0.05).(3)There were statistically significant differences between different TCM syndromes and patients’ GERD Q scores(P=0.033<0.05).(4)There were statistically significant differences between different TCM syndromes and endoscopic Yantai grade and bile reflux grade(P<0.05),but there was no statistical significance between different TCM syndromes and endoscopic mucosal manifestations(P=0.07>0.05).(5)There was statistical significance between GERD Q score and endoscopic Yantai grading(P<0.05),but there was no statistical significance between GERD Q score and endoscopic bile reflux grading(P=0.107>0.05).Conclusion:(1)In terms of gender,different TCM syndromes are related to the gender of patients.In the syndrome of liver and stomach stagnation and heat syndrome,the proportion of male patients is significantly higher than that of female patients,while there is no significant difference between the remaining syndromes.(2)In terms of the history of tobacco and alcohol use,different TCM syndromes are related to long-term smoking and drinking,among which the proportion of people with liver and stomach stagnation and heat syndrome who are addicted to tobacco and alcohol is the highest.(3)In terms of complicated diseases,different TCM syndromes are related to complicated hyperlipidemia,among which patients with liver and stomach stagnation and heat syndrome and patients with central deficiency and qi inversing syndrome account for the largest proportion.(4)In GERD Q score,different TCM syndromes were correlated with GERD Q score.GERD Q score of patients with liver and stomach stagnation and heat syndrome was significantly higher than that of patients with central deficiency and qi inversing syndrome,while there was no significant difference between the remaining syndromes.(5)In the endoscopic distribution,different TCM syndrome types and Yantai classification and grade of bile reflux are related,the Yantai class 0 patients are mainly distributed in patients with liver and stomach stagnation and heat syndrome and patients with qi stagnation phlegm blocking syndrome,I level mainly in patients with liver and stomach stagnation,while Ⅱ,Ⅲ patients mainly concentrated in patients with central deficiency and qi inversing syndrome;Bile reflux in the classification,gallbladder heat invading stomach syndrome patients and the relationship between the most closely,were found in bile reflux Ⅰ,Ⅱ,Ⅲ patients.(6)GERD Q score was correlated with endoscopic grading in Yantai,and patients in Yantai Grade 0 had the highest GERD Q score.
