Objective: This study aims to observe the clinical efficacy of Shentong Zhuyu Decoction for the treatment of residual symptoms of lumbar intervertebral disc herniation after PELD,and to explore its application value and mechanism in the treatment of residual symptoms after PELD,so as to treat the residual symptoms of PELD clinically.for reference. Methods: This study selected 60 patients who were hospitalized in the Department of Spine,Ruikang Hospital,Guangxi University of Traditional Chinese Medicine from December 2019 to December 2020.All patients were required to be diagnosed as lumbar intervertebral discs based on a combination of imaging examinations, symptoms and signs.Herniation(LDH),and only a single-segment disease, and percutaneous endoscopic lumbar discectomy(PELD)during hospitalization.Numbers 1-60 were coded in the order of admission time, and two groups of random numbers were obtained using SPSS25 statistical software, which were designated as the observation group and the control group. After the operation, the two groups adopted the same nursing and treatment measures and instructing functional exercises. The observation group was given the Shentong Zhuyu Decoction Chinese medicine granules from the first day after the operation on the basis of the control group.Taken for two weeks, collect VAS score,JOA score, and ODI score data at five time nodes before and 1 day after surgery,1 week and2 weeks after surgery, and 1 month after surgery, and evaluate the efficacy of modified Macnab in 1 month after surgery.Standard evaluation of curative effect, the collected data are processed by SPSS25.0 statistical software, and the curative effect is evaluated based on the changes of the two groups of observation indicators. Results: 1.A total of 60 patients were enrolled in this study. The follow-up time was 1 month. There were 30 cases in the observation group with an average age of(48.50±13.55)years and an average duration of(19.00±9.17)months.The prominent segment was located at L4-5,The number of patients with L5-S1 were 17 and 13 respectively;the control group had 30 patients, with an average age of(49.53±14.06)years and an average duration of(20.50±10.57)months.Among them, the prominent segments were L4-5 and L5-S1 patients There are 18 cases and 12 cases respectively. After statistical testing, the two groups of patients had no statistically significant differences in gender, age, course of disease, surgical segment and other baseline data,as well as the VAS score,JOA score, and ODI score 1 day before surgery(P>0.05),and they were comparable.2.Intra-group comparison: The two groups of patients were statistically significant in VAS,JOA,and ODI scores 1 day before operation and 1 day after operation(P<0.05),and the above evaluation indexes of each observation time node after operation Compared with 1 day before operation, there were statistical differences(P<0.05).Compared with the day before operation,VAS score and ODI score showed a continuous downward trend at various time points after operation, and JOA score showed a continuous upward trend.3.Comparison between groups:There was no statistical difference in the VAS scores between the two groups before and 1 day after surgery(P > 0.05),but there were statistically significant differences 1 week,2 weeks and 1 month after surgery(P < 0.05).The VAS scores of the observation group(2.33±0.71,1.10±0.61 and 0.37±0.49)were lower than those of the control group(2.73±0.74,1.80±0.76 and0.77±0.63)at 1 week,2 weeks and 1 month after surgery. There was no significant difference in JOA scores between the two groups before surgery,1 day after surgery and 1 week after surgery(P > 0.05).The JOA scores of the observation group(22.93±2.36 and 25.03±1.94)2 weeks after surgery and 1 month after surgery were higher than those of the control group(21.30±2.89 and 23.40±2.63).The difference was statistically significant(P < 0.05).There was no significant difference in ODI scores between the two groups on the day before operation,1 day after operation, and 1 week after operation(P>0.05).The ODI score observation group(24.00±2.38 and 2 weeks after operation and 1 month after operation)19.33±2.54)was lower than the control group(25.33±2.32 and 21.57±2.67),the difference was statistically significant(P<0.05).4.Overall clinical efficacy: the overall excellent and good rate of observation group was 93.3%(excellent in 20 cases, good in 8 cases),and the overall excellent and good rate in control group was 90.0%(excellent in 17 cases, good in 10 cases),after rank sum test,P>0.05,The difference was not statistically significant. Conclusion:PELD has a definite effect in the treatment of lumbar disc herniation.Shentong Zhuyu Decoction is satisfactory in the treatment of residual symptoms of lumbar intervertebral disc herniation after PELD.It can benefit patients in terms of pain relief and improvement of lumbar spine function without obvious side effects.Rehabilitation after PELD is of positive significance and is worthy of clinical promotion.
