According to the "2020 Social Blue Book",China’s middle-income group continues to expand.At present,my country has formed the most populous middle-income group in the world.With the changes in medical models and disease spectrum,the challenges of population aging,housing,Under the influence of education and other factors,the health anxiety of middle-income groups has become increasingly prominent.In the context of "Healthy China",a new type of health evaluation index-health-related quality of life is used to analyze and evaluate the health status of middle-income groups and their influencing factors It is of great significance.Object:Grasp the current status and characteristics of the health-related quality of life of the middle-income group,clarify the effect of each influencing factor on the quality of life,and understand the impact of five common chronic diseases on the middle-income group’s health-related quality of life and quality-adjusted life years.On the basis of this,we put forward targeted suggestions to provide reference for the health protection and intervention of middle-income groups and reasonable resource allocation.Methods:Using the data from the sixth health service survey in Jiangsu Province,the internationally accepted income quintile was used to screen out middle-income groups for research.The EQ-5D-3L scale is used to evaluate the health-related quality of life of the middle-income group aged 15 years and above.Based on the health determinant model,the appropriate variables are selected and included in the model by combining relevant literature and the actual situation in my country.Perform binomial Logistic regression and Tobit regression on the overall health score to analyze the factors affecting the health-related quality of life of the middle-income group.At the same time,select five common chronic diseases according to the prevalence of chronic diseases in the middle-income group from high to low,and use the Chinese effect score The system converts its health utility value and compares the impact of various chronic diseases on the quality of life of the middle-income group and the loss of quality-adjusted life years.Results:The average overall health score of the middle-income group is 81.49 ±13.84 points,and the average health utility value is 0.921.Overall,health-related quality of life is good.Among the 5528 study subjects,76.94%had no problems in all five dimensions.Among them,the proportion of pain or discomfort was the highest,18.33%.According to the regression results,men,higher education level,and physical exercise are the protective factors for quality of life(OR>1),and ageing,unemployment,and chronic diseases are risk factors for quality of life(OR<1),and rural residents are more likely to There are problems in the dimensions of pain or discomfort,anxiety or depression,presenting the characteristics of risk factors.The prevalence of chronic diseases in the middle-income group is 51.81%calculated by the number of cases.25.86%of the middle-income group suffers from two or more chronic diseases at the same time.There is a state of "comorbidity",and the prevalence is in the top five from high to low.The common chronic diseases are hypertension,diabetes,disc disease,acute and chronic gastroenteritis,and cerebrovascular disease.After controlling urban and rural areas,gender and age,it was found that these five chronic diseases all caused varying degrees of damage to the quality of life of the middle-income group.(P<0.05),its health effect value decreased by 0.034,0.045,0.061,0.028,0.147 in turn,and the annual loss of 23.98,7.43,6.48,1.63,13.03 years for every 10,000 population quality-adjusted life years.Conclusion:Strengthen the health protection of key groups such as rural areas,women,and the elderly;optimize the allocation of resources,improve health equity,and focus on the health conditions and health service needs of the low-educated and unemployed;accelerate the spread of health and optimize the health behaviors of residents;strengthen chronic diseases Screening,improve chronic disease management and service levels.
