Study On The Diagnosis And Treatment Experience Of Functional Gastroenteropathy And The Rule Of Medication By Professor Zhu Fangshi

Objective:to collect and sort out Professor Zhu Fangshi’s medical records and clinical experience in the diagnosis and treatment of functional gastrointestinal diseases,summarize his diagnosis and treatment experience,and use data mining technology to summarize and analyze his medication rules,so as to provide reference and help for learning and inheriting his experience and clinical application.This study will make a positive meaning in inheriting the experience and methods of spleen and stomach diseases of famous traditional Chinese medicine.Methods:1.Follow the tutor to the clinic,record the tutor’s medical words in detail and completely,and summarize Professor Zhu Fangshi’s academic experience and clinical experience in the diagnosis and treatment of functional gastrointestinal diseases.2.To collect the prescriptions of Professor Zhu Fangshi for the treatment of functional gastrointestinal diseases in the expert outpatient department of Jiangsu Province Hospital on Integration of Chinese and Western Medicine from May 2020 to February 2021.3,The patient’s medical information(name,clinical time,diagnosis,therapeutic regimen,etc.)were recorded into the database.Data mining technology such as frequency analysis,cluster analysis,association rule analysis and other methods were used to summarize the regularity and characteristics of drug use of Professor Zhu Fangshi in functional gastrointestinal diseases.Results:1.General information:a total of 90 cases and 133 prescriptions were collected,including 36 males and 54 females.2.Disease classification:of the 90 patients,56(62.22%)were diagnosed as FD,34(37.78%)cases were diagnosed as IBS.3.Frequency analysis:in the 133 prescriptions collected this time,the total frequency of drugs was 1640 times,and a total of 115 traditional Chinese medicines were used.40 kinds of drugs were used more than 15 times,accounting for 80.12%of the total drug frequency.The descending order of the top 10 herbs was as follows:Pericarpium Citri Reticulatae,Rhizoma Pinelliae,Pericarpium perenne,Scutellaria baicalensis,Magnolia officinalis,Fructus aurantii Immaturus,Acorus tatarinowii,radix paeoniae rubra,Rhizoma alismatis and semen Raphani.According to the efficacy,40 high-frequency drugs can be divided into 13 categories.From high to low,they are Liqi drugs,Qingre drugs,Huashi drugs,Huatan Zhike antiasthmatic drugs,Xiaoshi drugs,Lishui Shenshi drugs,Buxu drugs,Huoxue Huayu drugs,Kaiqiao drugs,jiebiao drugs,Xiexia drugs,Pinggan Xifeng drugs and hemostatic drugs.4.Cluster analysis:cluster analysis of 40 high-frequency traditional Chinese medicines,four cluster combinations were obtained:the first group:Cyperus,Fructus aurantii,Radix Aconiti,Radix Aucklandiae,Fructus aurantii,Pericarpium Citri Reticulatae,semen plantaginis,Poria cocos,semen Coicis,Rhizoma alismatis,Rhizoma acori tatarinowii;the second group:rhubarb,semen cannabis,Fructus wulingzhi,stir fried malt,stir fried grain bud,liushenqu,Jineijin,semen Raphani,Rhizoma Coptidis,Semen Cassiae,Gardenia jasminoides,cortex moutan Peony,wine Scutellaria,calcined ochre;the third category:Ophiopogon japonicus,Atractylodes macrocephala,Radix Paeoniae Alba,Huoxiang,Atractylodes lancea,Magnolia officinalis,Peilan;the fourth category:Perilla frutescens,bitter almond,Pinellia ternata,Ligusticum chuanxiong,Curcuma,Corydalis yanhusuo,bupleurum.5.Association rules analysis:133 prescriptions with a total of 1640 frequency drugs were analyzed.The minimum condition support was set as 30%,the minimum rule confidence was set as 70%,and the maximum number of front items was set as 3.It is concluded that there are 16 association rules with the first number of items being 1.There are 16 association rules with the first number of 2.There are three association rules with three items in front.Conclusions:Through three years of participating in the outpatient service of tutor Zhu Fangshi,oral and psychological teaching,collecting and sorting Professor Zhu Fangshi’s medical records,combined with data mining technology,finally summed up professor Zhu Fangshi’s diagnosis and treatment experience and medication rules of functional gastrointestinal diseases as follows.1.The tutor’s understanding of the etiology and pathogenesis of FGIDS is that the disease is located in the spleen and stomach,which is closely related to the liver,gallbladder,lung and intestine,and the etiology is related to improper diet,emotional disorder and insufficient spleen and stomach endowment.The most fundamental pathogenesis is that the Qi mechanism of middle Jiao is unfavorable,and the spleen and stomach rise and fall dereliction of duty.The main pathological factors were dampness,food and fire,and the pathological products were water dampness and blood stasis.2.The tutor’s academic experience in treating FGIDS are as follows:① disease differentiation is the first,syndrome differentiation is the most important;② internal injury and miscellaneous diseases,pulse condition is carefully identified;③seeking the root of the disease,knowing the origin of the disease;④ disease and evil are difficult to solve,with accumulation in it;⑤deficiency and damage of viscera,with dredging and tonifying;⑥ mediation of Qi,with orderly rise and fall;⑦guiding according to the situation,evil has a way out;Ⅷ body and spirit are integrated,with both body and mind;and;3.The commonly used basic prescriptions for FGIDS are Sini Powder,Tongxie Yaofang,Buhuanjinzhengqi powder,Xiaochaihu Decoction and Baohe Pill.4.The commonly used drug pairs of tutors are:Peilan Shichangpu,jiuhuangqin Zexie,Mudanpi Chishao,Chenpi Zhishi,FABANXIA jiuhuangqin,Chenpi FABANXIA,Peilan Chishao,Chenpi Peilan,FABANXIA Houpu,etc5.Commonly used drugs for tutors:tangerine peel,Pinellia ternata,pelan,wine Scutellaria,Magnolia officinalis,Fructus aurantii,Acorus tatarinowii,radix paeoniae rubra,Alisma orientalis,semen Raphani.6.Medication characteristics:pay attention to regulating qi and activating blood circulation,removing dampness and turbidity,dispersing stagnation,eliminating food and stagnation.To sum up,this study summarizes the tutor’s view on the diagnosis and treatment of FGIDS,especially emphasizes the importance of pathological products in the process of disease,which has positive reference significance for clinical diagnosis and treatment of FGIDS.
