The Clinical Observation On External Application Of Tongmai Xiaoban Ointment At Renying (ST 9) In Treatment Of Carotid Artery Plaque

ObjectiveTo evaluate the effect of Tongmai Xiaoban Ointment in the treatment of carotid artery plaques by measuring the carotid intima-media thickness,total plaque scores,blood lipid level and TCM symptom scores before and after treatment.MethodsPatients with carotid artery plaques who met the inclusion criteria were selected from the outpatient and inpatient departments of encephalopathy and psychopathy department of Shenzhen Hospital of Traditional Chinese Medicine.They were randomly divided into two groups:treatment group(30 cases)and control group(30 cases),a total of 60 cases.The treatment group was treated with external application of Tongmai Xiaoban Ointment at Renying(ST 9).The control group was treated with conventional Western medicine(Aspirin and Atorvastatin calcium tablets).The course of study was 12 weeks.Carotid intima-media thickness,total plaque scores(Crouse score),4 items of blood lipid,TCM symptom scores and safety indexes were measured before enrollment and 12 weeks after treatment,respectively.All the data collected before and after treatment were statistically calculated with SPSS26.0 software,and the final conclusion was drawn.ResultsThere was no shedding in the two groups,and all 60 cases completed 12-week clinical observation.The statistical results were calculated as follows:1.Before treatment,there was no significant difference between the two groups in gender,risk factors and other aspects by Fisherx2 test(P>0.05),showing equilibrium;Before treatment,there were no significant differences in carotid intima-media thickness(IMT),Crouse score and blood lipid between the two groups(P>0.05),indicating that the two groups were comparable.Before treatment,there was no significant difference in TCM symptom scores between the two groups(P>0.05),indicating that the two groups were comparable.2.After treatment of various observation result as follows:(1)The aspect of carotid exceeds:Before and after treatment by the paired samples of Wilcoxon rank-sum test,the two groups after treatment of carotid artery IMT,the Crouse integral were significantly lower than before treatment(P<0.05),between two groups by the Mann-Whitney U rank-sum test,the group of Tongmai Xiaoban Ointment and the control group had no significant difference(P>0.05),two groups were equally effective;(2)Four aspects of blood lipid:Before and after treatment by the paired samples of Wilcoxon rank-sum test,4 items of the two groups after treatment the blood lipid were obviously improved(P<0.05),the TC,TG,LDL-C are reduced,HDL-C was increased,the group of Tongmai Xiaoban Ointment and the control group had no significant difference(P>0.05),in regulating lipid has no obvious difference between the two groups.(3)TCM symptom scores:after treatment,TCM symptom scores in both groups were significantly lower than before treatment.The total effective rate of the treatment group was 86.7%,while that of the control group was 66.7%.The efficacy of TCM syndromes in the treatment group was significantly better than that in the control group by Mann-Whitney U rank-sum test(P<0.05).3.Safety:Before and after treatment,the safety indexes of Tongmai Xiaoban Ointment group and control group showed no obvious abnormality.Conclusion(1)The external application of Tongmai Xiaoban Ointment at Renying(ST 9)can reduce the intima-middle thickness of carotid artery and the total plaque scores,effectively reduce the levels of total cholesterol,triglyceride and low density lipoprotein cholesterol,and increase the level of high density lipoprotein cholesterol,which is equivalent to the conventional treatment of carotid artery plaque by western medicine.(2)The external application of Tongmai Xiaoban Ointment at Renying(ST 9)can effectively reduce the traditional Chinese medicine symptoms of carotid artery plaque patients with the syndrome of mutual association of phlegm and blood stasis,improve the clinical symptoms,and the effect is obviously better than the conventional western medicine treatment of carotid artery plaque.(3)The external application of Tongmai Xiaoban Ointment at Renying(ST 9)treatment of carotid plaques curative effect is good,good security,its easy to use,apply to avoid stimulus to the gastrointestinal tract,gastrointestinal intolerance or worry about side effects of statins in patients with provides a safe and effective choice,can be used as an alternative to conventional western medicine treatment of carotid plaques.
