| Objective:The left liver volume of patients with right hepatocellular carcinoma complicated with cirrhosis and its percentage in standard liver volume were measured by 3D reconstruction technique to provide theoretical basis for patients with right hepatocellular carcinoma complicated with cirrhosis to tolerance right hemihepatectomy.Methods:1.Collect the clinical data of patients who underwent three-dimensional enhanced multi-slice spiral CT of the upper abdomen in our hospital from January 2013 to October 2020 due to different causes.A total of 80 cases were in the normal liver group(45 males and 35females).A total of 71 patients in the liver cirrhosis group(42 males and29 females)were divided into A,B,and C groups according to liver function,with 32 cases in the A group,21 cases in the B group,and 18 in the C group.A total of 78 patients in the right liver cancer combined with liver cirrhosis(50 males and 28 females)were divided into A and B groups according to liver function,46 cases in the A group,and 32 cases in the B group.A total of 40 cases of right liver cancer without cirrhosis(25 males and 15 females)were divided into A and B groups according to liver function,25 cases in the A group,and 15 cases in the B group.2.Import the CT data of all patients into the French Myrian XP-Liver three-dimensional reconstruction software for three-dimensional reconstruction,and calculate the total liver volume and the left and right half liver volumes(the right hepatic volume of patients with liver cancer does not include the tumor volume).3.Record the height and weight of each patient,bring into the standard liver volume formula to calculate the standard liver volume,and further obtain the percentage of the standard liver volume for the left and right hemilivers.Results:1.The left and right liver volumes of the right hepatocellular carcinoma complicated with cirrhosis group were 365.74±103.49 cm~3 and 622.40±183.88 cm~3,respectively.The standard liver volume ratios of the left and right livers were 30.28 ± 7.72% and 51.29 ± 13.26%,respectively.The percentage of left and right liver in standard liver body of grade A group was 32.13± 8.17%,53.95± 10.95%;And 27.63±6.22% and 47.46±15.40% in grade B group.2.In the right liver cancer group without cirrhosis,the left and right liver volumes were 409.67±68.07cm~3 and 708.12±162.70cm~3,respectively.The ratio of the left and right livers to the standard liver volume was 33.14±5.29% and 57.03±11.67%,respectively.In the grade A group,the percentage of the right and right livers to the standard liver body was: 34.51±5.05%,58.29±11.05%;In B-grade group,the values were 30.86±5.01% and 54.94±12.74%.3.In the liver cirrhosis grade A group,the left and right liver volumes were 457.04±36.63 cm~3 and 571.39±84.93 cm~3,respectively,and the percentage of the left and right livers in the standard liver volumes were 37.31±3.07% and 46.66±6.99%,respectively.The left and right liver volumes of liver cirrhosis grade B group were 372.42±48.56 cm~3 and 518.37±73.05 cm~3,respectively.The percentage of the left and right livers in the standard liver volume were 31.54±3.87% and43.95 ± 6.18%,respectively.Left and right liver volumes of cirrhosis grade C group were 268.99 ± 40.89 cm~3 and 409.66 ± 49.46 cm~3,respectively,and the percentage of left and right liver in standard liver volume were 23.40 ± 3.50% and 35.29 ± 4.65%,respectively.4.In the normal group,the left and right liver volumes were 784.40±84.20 cm~3 and 427.64±51.71 cm~3,respectively,and the percentage of the left and right livers in the standard liver volumes were 34.16±3.28% and 63.00±5.06%,respectively.Conclusion:1.In the early stage of hepatitis B liver cirrhosis,the volume of the left liver is significantly increased,but as the liver function classification increases,the volume of the left liver is significantly reduced.2.The left hepatic volume of patients with right hepatocellular carcinoma and cirrhosis and its percentage of the standard liver volume decrease gradually with the classification of liver function.3.The left hepatic volume of patients with right liver cancer and cirrhosis accounts for about30.28±7.72% of the standard liver volume,which is significantly lower than 33.14±5.29% of patients with right liver cancer and cirrhosis.4.Patients with right liver cancer and liver cirrhosis who underwent right hemihepatectomy have less than 40% of the remaining liver volume,and there is a risk of postoperative liver failure. |
