ObjectiveThe objective of this study was to investigate the status quo of resilience and post-traumatic stress disorder in patients with acute myocardial infarction and its related influencing factors. To analyze the correlation between resilience and post-traumatic stress disorder; To explore the application effect of nursing intervention based on solution focused approach in patients with acute myocardial infarction. This study will has important practical significance for further expanding the research field and practical application of solution focused approach.Methods1.A total of 210 acute myocardial infarction patients who met the inclusion criteria from the five community health service centers in Hengyang City were selected as the study subjects from October 2019 to August 2020.The general information questionnaire,Connor-Davidson Resilience Scale and Post-traumatic Stress Disorder Checklist-civilian Version were used as research tools to investigate the 210 participants.Finally,203 valid questionnaires were collected with an effective recovery of 96.67%.2.Two community health service centers were randomly selected from the five community health service centers in the cross-sectional survey, each community health service centers was randomly assigned as a unit.Patients with acute myocardial infarction meeting the inclusion criteria were selected from the two community health service centers as the subjects in the intervention group and the control group, respectively, the number of each group of 42 patients. Two patients in each group withdrew from the intervention due to loss of follow-up,active withdrawal or exacerbation of the disease.Finally,40 patients in the intervention group and 40 patients in the control group completed the study.Patients in both the intervention group and the control group were given conventional nursing measures for acute myocardial infarction, and in addition, patients in the intervention group were also given nursing intervention based on the focused resolution approach. The general data, the scores of ConnorDavidson Resilience Scale and Post-traumatic Stress Disorder Checklistcivilian Version of patients with myocardial infarction in the intervention group and the control group were compared.3.Data were analysed using SPSS 18.0 software.Statistical methods including descriptive statistics,one-factor ANOVA,multiple linear regression analysis,Pearson correlation analysis,t-test and chi-square test.Results1.The total scores of resilience of participants were(61.21±5.79)points,the score ranged from 45 to 77.Education level, family relationship and the number of combined chronic diseases were the main influencing factors, which explained 28.9% of the total variation.2.The total score of post-traumatic stress disorder of participants were(39.96±4.88),the score ranged from 26 to 53.Education level and cardiac function grade were the main influencing factors, which explained 37.7%of the total variation.3.There was a negative correlation between resilience and posttraumatic stress disorder in the participants(r=-0.781,P < 0.01).4.Before the intervention, there were no significant differences in general data, resilience score, and post-traumatic stress disorder score between the intervention group and the control group(P > 0.05).5.After 6-week of the intervention, the total score of resilience and the score of tenacity dimension in the intervention group were significantly different from those in the control group(P < 0.05).Moreover,after 6-week of the intervention, compared with before intervention, there were statistical differences in the total score of resilience and scores of its dimensions, total score of post-traumatic stress disorder, score of reexperience dimension, and score of avoidance or numbness dimensions in the intervention group(P < 0.05).6.After 12-week of the intervention, the total score of resilience and scores of its dimensions in the intervention group were significantly higher than those in the control group, and the difference was statistically significant(P < 0.05);The total score of post-traumatic stress disorder scores and scores of its dimensions in the intervention group were significantly higher than those in the control group, and the difference was statistically significant(P < 0.05).8.After 12-week of the intervention, the total score of resilience and scores of its dimensions in the intervention group were significantly higher than before intervention, and the difference was statistically significant(P< 0.05);The total score of post-traumatic stress disorder and scores of its dimensions in the intervention group were significantly higher than before intervention, and the difference was statistically significant(P < 0.05).Conclusions1.The resilience of acute myocardial infarction patients was at a low level.Gender, education level, personal monthly income, payment method of medical expenses, family relationship, onset time, number of complicated chronic diseases and cardiac function grade were the influencing factors of resilience, among which education level, family relationship and number of complicated chronic diseases were the main influencing factors.2.The post-traumatic stress disorder of acute myocardial infarction patients was at a medium level.Education level, personal monthly income, cardiac function grade, onset time, number of chronic diseases and gender were the influencing factors of post-traumatic stress disorder, among which education level and cardiac function grade were the main influencing factors.3.Resilience of acute myocardial infarction patients was negatively correlated with post-traumatic stress disorder.4.The nursing intervention program based on the focused solution approach could significantly improve the resilience level of acute myocardial infarction patients and reduce the level of post-traumatic stress disorder.
