Objective Objective to investigate the medication compliance and blood glucose control of patients with type 2 diabetes mellitus in a tertiary hospital in Tianjin;to explore the influence of comprehensive nursing intervention based on MMC on medication compliance,blood glucose control effect,general self-efficacy and quality of life of patients with type 2 diabetes.Methods This study is mainly divided into two parts:current situation investigation and quasi experimental research.The first part:cross-sectional study,using convenience sampling method,270 patients with type 2 diabetes who were first diagnosed in a tertiary hospital in Tianjin from September 2018 to February 2019 were selected as the research objects.The general information questionnaire and 8 items of the Chinese version of Morisky(MMAS-8,Morisky medication adherence Scale-8 item)were used to investigate the medication compliance and its influencing factors.The second part:experimental study,according to the inclusion and exclusion criteria,a total of 160 patients with type 2 diabetes were selected,80 in the intervention group and 80 in the control group.After the informed consent was signed,the control group was given routine nursing intervention,and the intervention group was given comprehensive nursing intervention based on MMC.The medication compliance and blood glucose control effect of the two groups were followed up.The medication compliance,fasting blood glucose,blood lipid,glycosylated hemoglobin,BMI and other related indicators were collected and compared at 3 and 6months after the intervention,and the longitudinal change trend was observed,The intervention effect was evaluated.SPSS19.0 was used for statistical analysis.Mean ± standard deviation was used to describe the measurement data,frequency and percentage were used to describe the count data.Two independent sample t-test was used to compare the measurement data between the two groups.ANOVA was used to compare the measurement data between the two groups,and X2-test was used to compare the count data between the two groups.Logistic regression analysis was used to explore the main influencing factors of medication compliance No.Repeated measurement analysis of variance was used to compare the intervention effect between the two groups,P < 0.05 means the difference is statistically significant.Results1.The current situation and influencing factors of medication compliance of type2 diabetic patients.The overall medication compliance of diabetic patients was poor.The average score of MMAS-8 was(5.13 ± 2.40).51.7% of the patients had good medication compliance,48.3% had poor medication compliance,and the proportion of patients with high medication compliance reached the standard of blood glucose control.Logistic regression analysis showed that medication compliance was mainly affected by age,education level and blood glucose monitoring.2.The effect of comprehensive nursing intervention based on MMC.2.1 Comparison of baseline data:there was no significant difference in general information,disease-related data and compliance score between the two groups(P >0.05),and they were comparable.2.2 Comparing the change of medication compliance score with time between the intervention group and the control group,the results showed that:the change trend of medication compliance score and medication compliance score of type 2 diabetic patients in the two groups with time was inconsistent(P < 0.05),that is,the longer the intervention time,the faster the score of medication compliance in the intervention group increased.The results showed that the score of medication compliance in the intervention group was higher than that in the control group at 3 months and 6 months after intervention,and the difference was statistically significant(P < 0.05).2.3 Comparing the changes of fasting blood glucose with time in the intervention group and the control group,the results showed that:the intervention effect of fasting blood glucose in the two groups of patients with type 2 diabetes was inconsistent with the change trend of fasting blood glucose with time(P < 0.05),that is,the longer the intervention time,the faster the fasting blood glucose of patients in the intervention group decreased.The results showed that the fasting blood glucose of the control group was higher than that of the intervention group 3 months and 6 months after the intervention,the difference was statistically significant(P < 0.05).2.4 Longitudinal comparison of FBG,Hb A1 c,TC,TG,BMI of patients before intervention and 6 months after intervention,the results showed that:after 6 months of intervention,the decrease of FBG,Hb A1 c,TG,TC in the control group was lower than that in the intervention group,the difference was statistically significant(P < 0.05),but there was no significant difference in BMI between the two groups(P > 0.05).Conclusions1.The medication compliance of patients with type 2 diabetes is poor,which is still a health problem that needs to be paid attention to.The main factors influencing the medication compliance of patients with type 2 diabetes were age,education level and blood glucose monitoring.2.The comprehensive nursing intervention based on MMC has a positive effect on medication compliance and blood glucose control effect of diabetic patients.
