| Objective:To study the correlation between the syndrome differentiation of reflux esophagitis and the manifestations under gastroscopy, summarize and condense the rules of RE syndrome differentiation under endoscopy, with a view to increase the objective basis for TCM syndrome differentiation of RE;To study the correlation between TCM syndrome differentiation of RE and anxiety and depression,so as to further deepen the understanding of TCM on the etiology and pathogenesis of RE,in order to provide reference for clinical treatment of RE.Methods:Collect 180 patients who were treated at Fuzhou Hospital of Traditional Chinese Medicine from February 2020 to February 2021,who were RE by gastroscopy and met the criteria of TCM syndrome differentiation, and collected and sorted their age, gender, body mass index, and LA classification of esophageal mucosa under gastroscopy,Helicobacter pylori infection, microscopic comorbidities,Hamilton Anxiety Scale scores and Hamilton Depression Scale scores, and use SPSS23.0 to analyze the data.Results:1.General data analysis:In terms of age and gender, the ratio of male to female is about 1.43:1,with 46-55 years old as the peak incidence;in BMI,overweight patients account for the largest proportion, accounting for about 46.1%of the total number. The different syndrome types of RE patients are not significantly related to age(P>0.05),but there are significant differences in gender composition and BMI distribution(P<0.01).Among them, the syndrome of ganweiyure is dominant in men,and the syndrome of qiyutanzu in women Mainly, patients with piweishire syndrome have the highest BMI index.2.Distribution of TCM syndrome types:ganweiyure syndrome(18.9%)>piweishire syndrome(17.8%)>qiyutanzu syndrome(17.2%)>zhongxuqini syndrome(15.5%)>danrefanwei syndrome(15.6%)>yuxuezuluo syndrome(15%).3.Correlation between TCM Syndrome Types and General Information:The different syndrome types of RE patients are not significantly related to age(P>0.05),but there are significant differences in gender composition and BMI distribution(P<0.01).Among them, the syndrome of ganweiyure is dominant in men,and the syndrome of qiyutanzu in women Mainly, patients with piweishire syndrome have the highest BMI index.4.The correlation between TCM Syndrome types and gastroscopy results:①There is a significant difference in the LA classification among the different types of RE syndromes(P<0.01).The number of patients with piweishire in grade A was 65.6%,and in grade B,ganweiyure syndrome and danrefanwei increased most obviously, about 55.9%and 57.1%.②There are significant differences in Hp infection among different types of RE(P<0.05).The positive rate of HP was the highest in patients with piweishire.5.Correlation between TCM syndrome types and anxiety and depression states:①There is a significant correlation between RE syndrome and whether patients are combined with anxiety and depression(P<0.01).Among them, the proportion of patients with qiyutanzu syndrome and ganweiyure syndrome with anxiety and depression are significantly higher than the other four syndrome types.②The anxiety and depression levels of different TCM syndrome types of RE patients are significantly different(P<0.01).Among them, the HAMA and HAMD scores of patients with qiyutanzu syndrome and ganweiyure syndrome are both Significantly higher than the syndrome of danrefanwei syndrome, piweishire syndrome, yuxuezuluo syndrome, and zhongxuqini syndrome.Conclusions:1.Assessing the LA classification and Hp infection of RE patients helps to provide an objective basis for TCM syndrome differentiation.①There is a significant difference between the different TCM syndrome types of RE and the LA classification(P<0.01).As TCM syndrome types change from actual to deficient, the severity of esophagitis changes from mild to severe.② There are significant differences in Hp infection among the different TCM syndromes of RE,and the positive rate of Hp in the spleen and stomach damp-heat syndrome is the highest.2.The anxiety and depression states of different TCM syndromes of RE are significantly different(P<0.05).Among them, patients with liver and stomach stagnation-heat syndrome and qi stagnation and phlegm block syndrome have a tendency to anxiety and depression.Studying the relationship between RE TCM syndromes and anxiety and depression states will help to achieve early detection and early intervention in clinical practice for RE patients with anxiety and depression states,in order to improve the efficacy of RE. |
