| Objective:To seek a more optimal protocol for the treatment of herpes zoster neuralgia of stagnated heat of liver meridian,which is between the acute and posterior neuralgia phases,by comparing the clinical efficacy of plum blossom acupuncture andbloodletting combined with external application of polygonum perfoliatum L and oral administration of pregabalin,and to provide some reference for clinical treatment.Methods:68 patients with herpes zoster neuralgia of stagnated heat of liver meridian diagnosed in the Department of acupuncture and dermatology of the second people’s Hospital Affiliated to Fujian University of traditional Chinese medicine from June 2020 to January 2021 were collected.The patients were randomly divided into two groups.On the premise of the same life style education,the treatment group was treated with plum blossom needle tapping combined with external application of polygonum perfoliatum L.The control group was treated with pregabalin orally.Two groups are 10 days for a course of treatment,a total of 1course of treatment.The VAS score,the liver heat stasis symptom score,TCM syndrome score and the blood collection results of CD4,CD8 and CD4/CD8 after the first and the last 1days of treatment were recorded and recorded.The therapeutic effect was evaluated after treatment.The incidence rate of neuralgia after follow-up was 2 weeks after treatment.The data were analyzed by statistical software SPSS22.0,and the difference between the two groups was statistically significant.Results:1.VAS score: before treatment,there was no significant difference between the groups(P>0.05);before and after treatment,the differences were statistically significant(P<0.01);after treatment,the treatment group was better than the control group,the difference was statistically significant(P<0.05).2.TCM syndrome score: before treatment,the difference was not statistically significant(P>0.05);before and after treatment,the difference was statistically significant(P<0.01);after treatment,the treatment group was better than the control group,the difference was statistically significant(P<0.05).3.CD4,CD8,CD4/CD8: before treatment,there was no significant difference in CD4,CD8,CD4/CD8 between the two groups(P>0.05).After treatment,compared with before treatment,the levels of CD4 and CD4/CD8 in the two groups were increased(P<0.05),while the level of CD8 was decreased(P<0.05).After treatment,the level of CD8 in the treatment group was significantly lower than that in the control group,and the levels of CD4 and CD4/CD8 in the treatment group were significantly higher than those in the control group(P<0.05).4.Clinical efficacy comparison: the total effective rate of the treatment group was 93.75%,and that of the control group was 84.85%,the difference was statistically significant(P<0.05).5.Comparison of incidence rate of postherpetic neuralgia:the incidence rate of incidence of PHN in treatment group was 3.12%,while incidence rate of PHN in controlgroup was 24.24%,and incidence rate of PHN in treatment group was much lower than that in control group(P<0.05).Conclusion:1.The efficacy of plum blossom acupuncture and bloodletting combined with external application of polygonum perfoliatum L therapy in the treatment of herpes zoster neuralgia with stagnated heat of liver meridian was confirmed.Compared with oral pregabalin alone,it could better reduce the VAS score and Chinese medical evidence score and improve the CD4,CD8 and CD4/CD8 levels of patients.2.The efficacy of plum blossom acupuncture and bloodletting combined with external application of polygonum perfoliatum L in the treatment of herpes zoster neuralgia with stagnated heat of liver meridian is better than that of oral pregabalin alone. |
