Objective: Study the changes in neonatal heart rate,blood oxygen saturation,painful face,crying and other changes caused by pain during neonatal heel blood collection,explore the effects of white noise and mother’s voice on alleviating the pain of neonatal heel blood collection,and provide a basis for the implementation of voice intervention in neonatal pain management.Method: This research is a kind of quasi-experimental research.Convenience sampling.165 hospitalized newborns who underwent heel blood sampling in a large-scale tertiary general hospital in Xi’an from July to December 2020 were select research subjects and randomly divide them into 3 groups:the conventional group,the white noise group,and the mother’s voice group,with 55 newborns in each group.The routine group was set as the control group,and received routine care.The white noise group and the mother’s voice group were set as the experimental group,and pre-sound intervention is carried out on the day the newborn is admitted to the hospital,Place the recorder about 20 cm away from the newborn’s ears,and play the sound with a sound intensity of 55 d B,twice a day(9:00-10:00/14:00-15:00),10 minutes each time,3consecutive days.Intervention during the blood collection of newborns on the 4th day:the sound was played 1 min before the heel blood collection,and the sound was stopped4 min after the blood collection.The white noise group played the white noise "Colic Baby: White Noise for Babies" that was similar to the voice in the womb of a pregnant woman;the mother voice group played the recorded mother’s voice.Adopt the Neonatal Infant Pain Scale(NIPS),the Neonatal Facial Coding System(NFCS),By observing the changes in the neonatal heart rate,blood oxygen saturation,and the Changes in the first appearance time,first duration,and total duration of crying/painful facial features,to evaluate the impact of different voices on neonatal pain.Used excel to build a database,and SPSS 22.0 for analyzed.Statistical analysis methods such as statistical description,chi-square test,analysis of variance,and non-parametric test.Results:1.Comparison of neonatal pain scores:during blood collection and after blood collection(1min),Comparing the pain scores of the three groups of newborns based on NIPS/NFCS:the control group were statistically different from compared with the white noise group and the mother’s voice group(p<0.017);the white noise group compared with the mother’s voice group was no statistically significant(P>0.017).During the blood collection,the NIPS score/NFCS score control group was 5 points/5 points,the white noise group was 4 points/4 points,and the mother voice group was 4 points/4points;after the blood collection(1min),the NIPS score/NFCS score.The control group2 /4,the white noise group 0/1,and the mother voice group 0/0.2.Comparison of neonatal pain facial indicators:Comparison of the first appearance time,first duration,and total duration of the three groups of neonatal pain facial features:the control group were statistically different from compared with the white noise group and the mother’s voice group(P<0.017);In the first duration of pain,the white noise group Compared with the mother’s voice group was a statistically significant difference(P<0.017).In the first appearance time and total duration of the pain,the white noise group compared with the mother’s voice group was no statistically significant difference(P>0.017).First appearance time of painful face/first duration/total duration:the control group 2s/28s/100 s,the white noise group 4s/9s/60 s,the mother voice group5s/4s/50 s.3.Comparison of neonatal crying indicators:The first appearance time,first duration,and total duration of neonatal crying in the three groups were compared,the control group compared with the white noise group and the mother’s voice group were statistically significant difference(P<0.017);the white noise group Compared with the mother’s voice group was no statistically significant difference(P<0.017).Newborn crying first time/first duration/total duration:the control group 2s/30s/100 s,the white noise group 6s/7s/50 s,the mother’s voice group 7.5s/4.5s/40 s.4.Comparison of neonatal heart rate:Compare the average heart rate during the blood collection process and at the end of the blood collection 1-4 minutes,peak heart rate and the time when the heart rate returned to the baseline value :the control group Compared with the white noise group and the mother’s voice group were statistically difference(P<0.05);in the blood collection process and the time when the heart rate returned to the baseline value,the white noise group compared with the mother’s voice group was statistically significant difference(P<0.05/0.017);in After blood collection(1-4min),the heart rate peaked,white noise group compared with the mother’s voice group was no statistically significant difference(P>0.05).The highest peak of neonatal heart rate:the control group 188.47±22.18,the white noise group 178.53±18.70,the mother voice group 179.65±16.81;newborn heart rate return to baseline value:the control group 150 s,the white noise group 105 s,the mother voice group 90 s.5.Comparison of neonatal blood oxygen saturation: compare the average blood oxygen saturation,the lowest value of blood oxygen saturation,and the time for blood oxygen saturation to return to the baseline level during and after blood collection(1-4min):During and after blood collection(1-3min)blood oxygen saturation and its return to baseline level,the control group were statistically different from compared with the white noise group and the mother’s voice group(P< 0.017);the white noise group was no statistically different from compared with the mother’s voice group(P>0.017).between the three groups of newborns after blood sampling(4min)and its lowest value:there was no significant difference(P>0.05).the lowest value of blood oxygen saturation:the control group 91%,white noise group 94%,the mother voice group 93%;the time for the blood oxygen saturation to return to the baseline level:the control group 45 s,the white noise group 30 s,the mother voice 30 s.In conclusion:1.During heel blood sampling,newborns Will appear to varying degrees of increased heart rate,decreased blood oxygen saturation,crying,painful facial features,and pain scores.2.White noise and mother’s voice can reduce the pain score(NIPS/NFCS)during the heel blood collection process and recovery period of newborns;extend the time to the first appearance of painful face and crying from heel blood collection,shorten the first duration and total duration;reduce newborns The fluctuation range of heart rate rise;reduce the drop range of neonatal blood oxygen saturation;shorten the time it takes for neonatal heart rate and blood oxygen saturation to return to the baseline level.3.The mother’s voice group had better effects than the white noise group in shortening the first duration of the newborn’s painful face,reducing the fluctuation range of the heart rate during the blood collection process,and shortening the time it takes for the heart rate to return to the baseline level.
