ObjectiveThrough the study of the influence of white noise interference method combined with non-nutritional sucking on the pain and physiological indicators of newborn artery blood collection,to seek a simple and effective non-drug intervention program in newborn pain management,which provides experimental basis for the effect of neonatal non-drug analgesia intervention and promotes the implementation of non-drug analgesia measures for newborn.MethodsA total of 105 neonates hospitalized in neonatal intensive care unit of a Grade-III hospital in Qingdao from April 2022 to June 2022 were randomly divided into 3groups by simple random sampling method:Control group,non-nutritional sucking group(NNS group)and white noise combined non-nutritional sucking group(WN+NNS group),with 35 neonates in each group.The control group neonates were given routine nursing care during arterial blood collection.The NNS group was given non-nutritive sucking(sucking pacifier)at the same time of routine nursing care;The WN+NNS group was given non-nutritional sucking with a pacifier while playing white noise during routine nursing care.The measures taken by the three groups were from 60s before arterial blood collection to 120s after arterial blood collection.Neonatal Pain Assessment Scale(NIPS)and Preterm Infant Pain Profile-Revised(PIPP-R)were used to evaluate the pain degree of the three groups.COMFORTneo Scale was used to evaluate the comfort level of infants.Changes in physiological indicators such as heart rate and SPO2were recorded before,during and after blood collection.Recorded the number of cries,the duration of cries,the duration of the painful face and time to sleep after blood collection.Import the data into the SPSS 26.0 software for statistical analysis,use statistical methods such as t-test,chi square test,Kruskal-Wallis rank sum test for comparison.Results1.There was no significant difference in the general data among the three groups(P>0.05).2.There was no significant difference in NIPS score and PIPP-R score among the three groups before arterial blood collection(P=0.267,P=0.211).The NIPS scores and PIPP-R scores in NNS group and WN+NNS group were lower than those in the control group during arterial blood collection,60s after blood collection and 120s after blood collection(P<0.001).The PIPP-R scores in WN+NNS group were lower than those in NNS group during arterial blood collection,60s after blood collection and 120s after blood collection(P<0.05).3.There were statistically significant differences in the comfort of children in three groups.The scores of COMFORTneo Scale of children in the NNS group and WN+NNS group were lower than those in the control group(P<0.05);the scores of COMFORTneo Scale in the WN+NNS group were lower than those in the NNS group(P<0.05).4.(1)Compared with before arterial blood collection,neonates in control group and NNS group showed increased heart rate during blood collection to 120s after blood collection(P<0.05);In WN+NNS group,the heart rate increased only in 60s after blood collection(P<0.05).(2)There was no significant difference in heart rate of neonates between NNS group and control group at different stages(P>0.05).The heart rate of WN+NNS group was lower than that of control group and NNS group during blood collection,60s after blood collection and 120s after blood collection(P<0.05).(3)The maximum heart rate and heart rate fluctuation range in WN+NNS group was lower than that in control group(P<0.05).5.There was no statistical significance in blood oxygen saturation and the minimum value of blood oxygen saturation among three groups(P>0.05).The fluctuation range of blood oxygen in NNS group and WN+NNS group was lower than that in control group(P<0.05).6.The number of Crying in WN+NNS group is lower than that in control group and NNS group,the difference was statistically significant(X2=15.238,P<0.05;X2=4.629,P<0.05).7.The duration of crying,duration of painful face and time to sleep after blood collection in NNS group and WN+NNS group were lower than those in control group(P<0.05);the duration of crying,duration of painful face and time to sleep after blood collection in WN+NNS group were lower than those in NNS group(P<0.05).Conclusion1.Non-nutritional sucking and white noise combined with non-nutritional sucking during arterial blood collection can reduce the pain degree during and after arterial blood collection,shorten the duration of crying and duration of painful face,promote sleep,and white noise combined with non-nutritional sucking can reduce the fluctuation of vital signs during arterial blood collection;2.The combined application of non-nutritional sucking with white noise can relieve the pain of neonatal arterial blood collection,and the effect is better than that of non-nutritional sucking alone.The combination of white noise and non-nutritive sucking is recommended for pain management of neonatal arterial blood collection.
