| ObjectiveObjective To observe the clinical efficacy of Sanhuang Xiao Cuo mask on patients with moderate acne and the effects of auricular acne on the skin lesions,pathology and serum IGF-1 and DHEA levels,and to explore the therapeutic effect and possible mechanism of the treatment.Method1.Clinical observation: The clinical trial cases were divided into the control group and the treatment group.The control group was treated with Metronidazole Gel,sooner or later each time.The treatment group was treated with The Sanhuang Xiao Cuo mask once a night.The patients in both groups were asked to wash their faces with warm water in the morning for 4 weeks.During the treatment,both groups were treated with Pipa Qingfei drink.The patients were evaluated with Gags score,TCM syndrome score before and after treatment,effective rate of each group,and adverse reactions during treatment were recorded.2.experimental research: Rats in animal experiment were divided into blank group,model group,control group,Sanhuang Xiao Cuo mask high,medium and low dose group.In addition to the blank group,the other groups were made auricle compound acne model for 21 days.22 d began treatment.Blank group and model group were not given topical medication.The control group was treated with Metronidazole Gel,sooner or later.The Sanhuang Xiao Cuo mask was given high dose,medium and low dose groups once a day.The treatment lasted for 28 days.24 hours after the last administration,blood was taken from the heart of rats in each group after anesthesia,and the supernatant was centrifuged.The contents of IGF-1 and DHEA were measured by ELISA.The pathological changes of the right auricle were observed under microscope and the experimental results were analyzed.Result1.Clinical observation(1)Gags score comparison: after treatment,the Gags score of the two groups decreased compared with that before treatment,and the difference was statistically significant(t = 6.70,P = 0.01< 0.05).The improvement of facial skin lesions in the treatment group was better than that in the control group.(2)Comparison of TCM syndrome score: the difference between the two groups was statistically significant(t =-2.46,P = 0.02 < 0.05).The improvement of TCM syndrome score in the treatment group was better than that in the control group.(3)Comparison of effective rate: the effective rate of the control group was 72.22%,and that of the treatment group was 85.29%.The effective rate of the two groups was statistically different by rank sum test(z =-2.08,P = 0.04 < 0.05).The curative effect of the treatment group was better than that of the control group.(4)The incidence of adverse reactions: the incidence of adverse reactions in the control group was 16.67%;the incidence of adverse reactions in the treatment group was 14.71%.By rank sum test,the incidence of adverse reactions in the two groups was not statistically significant(z =-0.22,P = 0.82 > 0.05).2.Experimental study(1)the changes of skin lesions were seen in the naked eye of each group.Compared with the model group,the control group and the Sanhuang Xiao Cuo mask had higher erythema,less desquamation and softer texture in the high,middle and low dose groups.The improvement degree of skin lesions in mask group was positively correlated with the dose of medicine.(2)the pathological changes of the ear lesions in each group: compared with the auricle of the model group,in the control group,the Sanhuang Xiao Cuo mask high,middle and low dose group,the inflammatory cells infiltration in the auricle of the rats decreased,the accumulation of keratinized substances in the sebaceous duct opening of the hair follicles decreased,and the stratum corneum became thinner.The pathological improvement degree of the facial mask group was positively correlated with the dose of the drug,and the pathological improvement degree of the rats in the high dose group was the best.(3)the changes of serum IGF-1 and DHEA contents in each group: compared with the model group,the levels of IGF-1 and DHEA in the serum of the control group,Sanhuang Xiao Cuo mask,high,medium and low dose groups were all decreased,and the difference was statistically significant(P < 0.05).The difference was statistically significant(P < 0.05).ConclusionClinical study compared the Gags score,TCM syndrome score,effective rate and incidence of adverse reactions between the two groups of Sanhuang Xiao Cuo mask and Metronidazole Gel,and confirmed the effectiveness of the two groups in clinical treatment,and the treatment group was better than the control group.The effects of Sanhuang Xiao Cuo mask on skin lesions,pathology,serum IGF-1 and DHEA contents in the auricular compound acne model rats were compared.To sum up,Sanhuang Xiao Cuo mask is effective in treating moderate acne and has no obvious serious adverse reactions.Its improvement on moderate acne may be related to the regulation of IGF-1 and DHEA metabolic factors,which is worthy of further promotion in clinical and theoretical research. |
