| Objective:To explore and analyze the correlation between the systemic immune inflammation index(SII)and the prognosis of patients with alveolar hydatid disease.Methods:The clinical data of patients undergoing radical hepatic alveolar hydatid surgery admitted to the Department of Hepatobiliary and Pancreatic Surgery of Qinghai University Affiliated Hospital from January 2015 to December 2018 were retrospectively analyzed,and their SII values were calculated.Spearman correlation analysis was used for correlation analysis.The receiver operating characteristic curve(ROC)was used to determine the best clinical cut-off value of SII,and the patients were divided into two groups with low SII and high SII.The Kaplan-Meier method was used to draw the survival curve to determine the overall survival time of the two groups of patients The analysis was carried out,and log-rank test was used for univariate analysis of postoperative survival,and Cox regression proportional hazard model was used for multivariate analysis of clinical prognosis.Result:A total of 242 patients were enrolled,including 96 males and 146 females,aged 11.0-67.0(36.6±11.7)years old.The results of correlation analysis showed that SII was significantly correlated with postoperative survival status of patients with alveolar hydatid disease(r=0.267,P<0.001).The ROC curve was used to determine the optimal clinical cut-off value of SII before operation 758.92.The 242patients with alveolar hydatid were divided into low SII group(SII≤758.92)and high SII group(SII>758.92),with 126 cases and 116 cases.Using Kaplan-Meier survival curve to analyze the survival rates of patients with hepatic alveolar hydatid in the low SII group and the high SII group were 98.2%,88.47%,66.1%and 90.8%,53.05%,and27.4%,respectively.In the low SII group,the cumulative survival rate was>50%,and the average survival time was 55.584 months(95%CI:53.550~57.617);in the high SII group,the cumulative survival rate was<50%and the average survival time was 39.384months(95%CI:35.070~43.698),the median survival time is 43 months(95%CI:34.694~51.306),The survival rate of patients with hepatic alveolar echinococcosis in the low SII group was significantly better than that in the high SII group,The overall survival rate difference between the two groups was statistically significant(χ~2=46.979,P<0.05).Univariate analysis showed that SII>758.92(HR=5.907,95%CI:3.386~10.306)was a risk factor for the overall survival time of patients with alveolar echinococcosis.Cox multivariate analysis verified the preoperative peripheral blood SII(HR=3.507,95%CI:1.911~6.435)is an independent risk factor for the overall survival rate of patients with alveolar echinococcosis.Conclusion:The preoperative SII level has a clear correlation with the prognosis of patients with hepatic alveolar echinococcosis,which can be used as a clinical indicator to evaluate the prognosis of patients.The higher the SII in peripheral blood before surgery,the worse the prognosis of the patient. |
