Object:By measuring the changes of MPV and D-D levels in patients with different degrees of cardiac function in Xining area, the correlation between MPV and D-D levels and cardiac function was analyzed, and its relationship with the degree of cardiac function in HF patients was explored to provide a reference index for early diagnosis and treatment of HF.Methods:A total of 114 HF patients who were hospitalized to the Department of Cardiology of Qinghai University Affiliated Hospital from January2019 to October 2020 were enrolled as the HF group(all meeting the inclusion and exclusion criteria),In the same period,110 people with normal physical examination results in the same hospital were enrolled as the control group. The HF group was divided into class II,III,and IV groups according to NYHA functional classification, with 40,39,and 35 cases, respectively;it was divided into HFr EF,HFmr EF,and HFp EF groups according to LVEF,with 33,40,and 41 cases, respectively.General data were collected from all included subjects, and blood routine, biochemistry, coagulation and NT-pro BNP parameters were collected on an empty stomach from the next morning for cardiac color Doppler ultrasonography. The obtained data were statistically analyzed.Results:1.BMI,SBP,TG,MPV and D-D in HF group were higher than those in control group(P<0.05);There was no statistically significant difference between HF group and control group in terms of smoking, drinking history, gender, age, diastolic blood pressure, fasting blood glucose, total cholesterol, high and low-density lipoprotein, creatinine, urea nitrogen, uric acid, and platelet count(P>0.05).2.In HF patients,MPV levels were higher in NYHA functional class IV group than in class II group(P<0.05);D-D levels were higher in the grade IV group than in the grade III and II groups(P<0.05);There were no significant differences in the levels of MPV and D-D among the remaining groups(P>0.05).3.The MPV levels in the HFr EF and HFmr EF groups were higher than those in the HFp EF group(P<0.05);The D-D level in the HFr EF group was higher than that in the HFmr EF and HFp EF groups(P<0.05);there were no statistically significant differences in the levels of MPV and D-D among the rest(P>0.05).4.MPV and D-D levels were positively correlated with NYHA functional class(r_s=0.450,P<0.001;r_s=0.474,P<0.001),n e g a t i v e l y c o r r e l a t e d w i t h e j e c t i o n f r a c t i o n(r=-0.341,P<0.001;r=-0.441,P<0.001),a n d p o s i t i v e l y c o r r e l a t e d w i t h NT-pro BNP c o n c e n t r a t i o n(r=0.463,P<0.001;r=0.512,P<0.001).5.Logistic regression of HF risk factors revealed that SBP,TG,MPV,and D-D were risk factors for HF(P<0.05).Conclusion:1.The levels of MPV and D-D in HF patients in Xining area increased with the increase of NYHA functional classification and increased with the decrease of LVEF,which could reflect the cardiac function status of HF patients. The determination of the two indicators has certain guiding significance for early determination of cardiac function classification of HF patients.2.MPV and D-D levels were significantly positively correlated with NT- pro BNP concentrations, and were risk factors for HF.3.The abnormal levels of MPV and D-D in patients with HF indicate that patients with cardiac insufficiency have abnormal coagulation function, and the more severe the degree of cardiac insufficiency, the higher the level of both, indicating that patients with cardiac insufficiency may have hypercoagulable state, and intervention should be given as early as possible.
