In our country,the medical resources are limited,and the people’s demand for medical resources is increasing year by year.Supply can not meet the high demand is the main problem faced by hospitals,how to effectively solve optimization under conditions of limited medical resources has become critical.Operating room scheduling is an important part of optimizing the allocation of medical resources.A scientific scheduling plan can optimize the allocation of operating room resources,improve the efficiency and service quality of the operating room,and effectively alleviate the contradiction between supply and demand.In order to improve the efficiency of the operating room,some hospitals have begun to improve the traditional surgical process.By matching the resources of the operating room in more detail,the patient’s anesthesia is arranged in the operating room under the traditional process,and the improved process is performed on the patient by setting up a separate anesthesia room.This kind of non-operating room anesthesia strategy is intuitively believed to effectively improve the efficiency of the operating room,but the advantages and disadvantages of this strategy have yet to be studied.Under this strategy,the traditional operation scheduling problem has changed from considering only a single operating room scheduling to considering the two stages of anesthesia room and operating room,which makes the traditional operating room scheduling model not suitable for the scheduling problem under this strategy.Operating room scheduling mainly involves elective patients and emergency patients.Emergency patients usually arrive randomly and need to be operated as soon as possible.It is often difficult to effectively allocate resources,and will cause problems such as delayed,interrupted,and inefficient operating rooms.How to effectively deal with emergency patients is also a difficult part of hospital management.After emergency patients are inserted,there will often be a lot of negative effects.This factor will greatly affect the deployment of the scheduling plan and cause a lot of management costs.It is necessary to further consider reducing the interference caused by emergency patients.The rescheduling plan for surgery becomes an effective solution.Therefore,this article studies the surgical scheduling under the strategy of setting up an independent anesthesia room,analyzes the impact of this strategy on the efficiency of the operating room.The research involves elective patients and emergency patients.When emergency patients arrive randomly,consider generating a pre-dispatch plan for emergency patients,and further consider rescheduling in order to reduce the interference of emergency patients.The research is divided into two phases: the initial scheduling phase and the rescheduling phase.First,the first stage aims at maximizing the efficiency of the operating room,considering the random arrival of emergency patients.By controlling the overlap between operating rooms,the uncertainty is transformed into constraints and an initial scheduling model is constructed to generate a model that can deal with the random arrival of emergency patients pre-dispatch plan for elective patients.Then,in the re-dispatch stage,based on the first stage,further consider the impact of emergency patient arrival,and consider how to adjust the scheduling plan after the emergency patient actually arrives.This article believes that the arrival of emergency patients will force changes in the scheduling decisions in the initial plan,and will cause problems such as patient transfers and low operating room efficiency at the same time.Therefore,a rescheduling model with the goal of operating room efficiency and cost balance is constructed.This stage by adjusting the scheduling decisions of unstarted elective surgery to rationally optimize the allocation of resources,thereby reducing the negative impact of emergency patients and improving the efficiency of the operating room,the final surgical scheduling plan is generated.Numerical experiments show that this independent anesthesia room strategy can effectively improve the efficiency of the operating room and reduce the cost of the operating room,and the advantages become more obvious as the size of the operating room increases.In the re-scheduling stage,considering the trade-off scheme of operating room management and efficiency has more application value.In theory,the research further improves and enriches the operating room scheduling research in response to emergency patients and rescheduling from the perspective of surgical process improvement.At the practical level,this article considers the balance between hospital efficiency and management in the re-dispatch phase,and provides guidance and decision-making for hospital managers to respond to the arrival of emergency patients,adjust the scheduling plan to improve efficiency.After analysis,this problem is NP-hard,and it is difficult to solve effectively when the data scale is large.In this paper,two heuristic algorithms are designed to solve the two-stage model.The heuristic algorithm is based on the SPT-LPT scheduling rules,and some strategies are designed to effectively deal with emergency patients and adjust the scheduling plan.
